INSPECTION ID DATE DESCRIPTION COMMENT VIOLATION ID VIOLATION CODE VIOLATION COMMENT
-1033022294 2012-01-09 Equipment to arrive later today to start building the location. Poulson Drilling is scheduled for early next week.
-1215709114 2012-03-26 Bass Energy is filling four 500 bbl frac tanks from an Akron city fire hydrant. The perforating is scheduled for 3/27 and the frac for 3/28. No violations at this time.
1326990754 2012-01-17 The rig set and cemented 109' of 11-3/4 conductor casing was set and cemented yesterday. The job was not witnessed since the activity occurred on a holiday. According to the records left on site by the cement contractor (Universal) and interviews with Poulson employees, cement did not circulate to surface. Circulation was maintained during the job but cement did not reach surface. After displacing the plug, the ground surface started to fail around the well which made surface grouting impractical. Rig integrity was never in doubt. It appears that insufficient cement was mixed and displaced. Additional cement will be ordered for cementing surface casing.
1333979450 2012-03-28 Berry Well Service and Universal Well Service were on site to frac the well. The slickwater job included 70,000 gallons of water and sand. No violations at this time.
1465190061 2012-01-27 Circle C is on site to provide the solidification material and the equipment to start closing the drilling pits. Pander Trucking has been removing pit water either from a 300 bbl frac tank or directly from the pits.
1951648123 2012-01-18 Plug down on the surface casing cement job was at 22:36 on 1/17. On site this morning to witness the blowout preventer (BOP) test. The annular BOP test was successful. Poulson will resume drilling the main hole this morning. The pits are lined and in good condition.
2066609041 2012-01-25 Poulson Drilling has moved off location. The drilling pits are fenced, lined and in good condition and are being pumped down. No violations at this time.
-505161678 2012-01-20 Poulson is drilling ahead on fluid at 3040'. The pits are lined and in good condition. Poulson switched from air to fluid at 3142 as required by permit.
