1077965843 2015-01-21 On the date of inspection I was on site to witness the grout job for the 24 inch diameter surface casing. ODNR Inspector Andrew Corder was on site earlier in the day to witness the surface job but there was a small crack in a weld job near the well head that opened up during displacement. Rice Energy consultant, Greg Ritter, informed me that they had stopped the job and allowed the cement to balance itself out while they corrected the welding problem. Ritter was able to tag cement at 63 feet below ground using a string and weight. Ritter waited for four hours before grouting the backside of the 24 inch pipe to surface. Baker Hughes pumped 100sk of Class A, at 15.6ppg. 6bbl of cement returned to surface and stayed at surface while I was on site. Rice will wait 4 hours from pump time to run pressure test.
1421939098 2015-01-14 On the date of inspection I was on site to witness the cementing of the conductor. I was informed by Rice Energy consultant Fred Rice that Well Service Group had drilled a 36 inch diameter hole to 100 feet. They set 100 feet of 106lbs/ft, 30 inch diameter conductor pipe on bottom. Allied Oil & Gas Services attempted to establish circulation with fresh water before pumping cement and their pump truck pressured up. The job was temporarily shut down while the crane on site lifted the casing high enough for Allied to establish circulation. The crane then lowered the pipe until it washed down to bottom. Allied pumped 331sk of Class A cement, 15.6ppg, 1.19 yield. 3bbl of cement returned to surface.

1517568110 2015-01-21 On this day the 24 casing was cemented into a 28 open hole to a depth of 317'. Baker ran 513 sks of Class A cement with an excess of 30%. Cement to surface was established, however a weld on the top plate was compromised and a leak was noted. The leaked cause the cement to drop on the backside. The cement will balance out and a top off job will be preformed to ensure correct cement bond between the open hole and the 24 casing. No violations noted.
1611207756 2015-01-21 Cement on the backside of the 24 casing string was tagged after waiting a few hours for it to set up. Cement was tagged at 60' down the backside. Baker then remediated by grouting Class A cement from the point of 60' to surface. No violations noted.
1628177235 2015-01-26 Current operations are rigging down to transit off location.
