1557341994	2014-08-07		The operator has corrected the gas leak from the well. The surface casing was patched and a one half inch fitting was installed. The valve on the fitting is closed. The operator needs to run a corrosion survey on the well to determine the status of the production casing. The NOV from 7/17/14 is terminated.
-161818983	2014-08-21		Inspect well. Measure pressure on annulus at 10 psi. Measure pressure on 3 inch tubing at 10 psi, gate valve may be closed.
1686145872	2014-07-22		The operator cleared brush around the well. They attempted to repair the leak with a temporary rubber patch, however it is not sealed. The casing is rusted out and leaking gas. The well will need to be plugged if the casing cannot be repaired. The pumper was contacted via phone and advised that the well is still leaking. He is going to attempt to repair the 1 inch fitting on the 8 inch casing to thread a plug into it, however he was advised that the production casing needs to be inspected to determine its integrity. The amount of gas venting from the well indicates that the production casing has failed.
-2083105373	2014-07-17		The well is leaking gas from the production casing at the ground surface. The operator has failed to clear trees and brush around the well. The well tender was contacted and is on the way to the location. A violation is being issued for failure to contain formation pressure. Repair the well by August 17, 2014.
