1286474651 2010-10-06 Convert to SWIW inspection request from Tom Tomastik. Well site is located in a corn field. Corn crop is on 3 sides with the 4th side a wood line with pine trees and mixed hard woods. There are three water wells with in 1/2 mile (2640'). (1) Windon is 1240' South West of proposed Injection well. (2) Millhone is 1189' East of the proposed Injection Well. (3) Nutzling is 1550' East of the proposed Injection well. There are two intermittent streams (1) 1832' to West of proposed Injection well. (2) 2302' to East of the proposed Injection well. Production well has good identification. Well is equipped with 7 casing head, 4 1/2 casing head, with 1 1/2 tubing (valve on tubing is closed). Well has a 2 black plastic line that is connected to a 1100 gallon plastic tank (tank is empty). Well is not connected for gas production. No leaks or contamination observed during inspection. Well site has good vegetative cover established.
1316561633 2012-10-17 Topped off annulus with 2 barrels fresh water. Pumped annular pressure to 800 psi then pressure dropped rapidly. Pulled more tension on tubing and packer total of 26,000 lbs. Tested three aditional times using 15 gallons of fresh water. Unable to hold 880 psi for 15 minutes with no more than 5% loss. Pulled more tension on the packer 27,000 lbs. Tested at 910 psi for 15 minutes with no decline in pressure. Test passed.
1350416109 2012-10-16 Conversion to salt water injection well. Appalachian Well Surveys logged (Gamma Ray CCL) and perforated the 4.5 casing from 3728' to 3668' (60 shots). Wire line total depth was 3764'. Halliburton R-4 (rebuilt) Packer set at 2978' with new 2 3/8 R-2 10 round tubing. Packer ran with 3 shear pins. Packer set at 25,000 lbs (tubing string weight 12,500 lbs). Packer was set at 25,000lbs. Annulus was filled with 30 barrels fresh water and 4 gallons inhibitor. Unable to test annulus due to air in the annulus.
1350555475 2012-10-17 Well settled down over night. Water was added on top of the packer too quickly entrapping air. This caused the well to burp air and water. The operator decided to wait until the next day to continue the mechanical integrity test on the packer. Performed three failed mechanical integrity test at the beginning of the morning, reseating the packer after each test with increasingly higher pull pressure on the drill string and packer. The fourth reseating of the packer had a total pull weight on the drill string and packer of 27000 lbs, with approximately 14000 pounds of that applied to the packer. The maximum allowable injection pressure on this well was permitted at 880 psi. This last mechanical integrity test started at 910 psi and stayed at that pressure for 15 minutes. It passed.

I met tom Schoonover in the field and we noticed the production casing head had developed a small leak around the threads. 243547150 2012-10-16 Observed the conversion of this Ohio Shale production well to an injection well. Met Jon Scott at the well site. Appalachian Well Surveys was at the site waiting on JD Drilling personnel. JD Drilling service truck, flatbed truck with new 100 joints of 2-3/8 inch production tubing with a measured total length of 3030 feet,, and a UIC brine truck filled with fresh water were also parked around the wellhead. Appalachian logged the well with gamma ray-collar locator logs. Rigged up a 60-feet perforator string, one shot per foot and shot perfs at 3668 to 3728 interval to enhance injection capacity of brine combined with existing perforations in the upper Ohio Shale. JD used a refurbished Halliburton R4 Packer with three shear pins in place, each rated at 6600 lbs of shear before failure for a total of 19800 lbs. They lowered the packer into the hole to a depth of 2978.2 feet with 98 joints of the 100 on location. They tried to flush the perfs with some of their water but lost all of their supply due to vacuum down the hole. So they had to get a new supply of fresh water and Baker Hughes inhibitor to perform the mechanical annular space integrity test.. Using the tubing JR Drilling seated the packer at 2978.2 feet with 25,000 lbs total pull on drill pipe and packer with 12,500 lbs on packer. They poured their fresh water with inhibitor down the hole but not slowly. The well continued to burp air and water forcefully so they decided to let it settle over night. Shut down until tomorrow
