I collected a water sample and transported it to the lab in Cambridge.

They want to connect the tanks in the earthen dike to a pump and inject the fluid in the tanks, I told them to send a plan to the UIC section outlining their plan for operation of the facility. I reminded them that the compliance notice for contamination outside of a surface facility has a time line of 10 days. No contaminated soil has been removed from the site. Recent rain event is flushing fluid to the plastic lined pit.

0243547150 2012-10-16 Salt Water Injection Well No Violations Observed the conversion of this Ohio Shale production well to an injection well. Met Jon Scott at the well site. Appalachian Well Surveys was at the site waiting on JD Drilling personnel. JD Drilling service truck, flatbed truck with new 100 joints of 2-3/8 inch production tubing with a measured total length of 3030 feet,, and a UIC brine truck filled with fresh water were also parked around the wellhead. Appalachian logged the well with gamma ray-collar locator logs. Rigged up a 60-feet perforator string, one shot per foot and shot perfs at 3668 to 3728 interval to enhance injection capacity of brine combined with existing perforations in the upper Ohio Shale. JD used a refurbished Halliburton R4 Packer with three shear pins in place, each rated at 6600 lbs of shear before failure for a total of 19800 lbs. They lowered the packer into the hole to a depth of 2978.2 feet with 98 joints of the 100 on location. They tried to flush the perfs with some of their water but lost all of their supply due to vacuum down the hole. So they had to get a new supply of fresh water and Baker Hughes inhibitor to perform the mechanical annular space integrity test.. Using the tubing JR Drilling seated the packer at 2978.2 feet with 25,000 lbs total pull on drill pipe and packer with 12,500 lbs on packer. They poured their fresh water with inhibitor down the hole but not slowly. The well continued to burp air and water forcefully so they decided to let it settle over night. Shut down until tomorrow

The manifold was tested to the Conroy injection tubing master valve, the Showalter injection line valve on the facility side of the line, The injection line from dike A and the injection line from dike B. Tested at 880 psi at 5 minutes 855 at 15 minutes 830 psi. 1st test failed. A 2nd test at 825 psi, at 5 minutes 820 psi, at 15 minutes 815 psi. Test passed. JD Drilling Supervisor was informed that the test pressure of 825psi instead of 880 psi will result in the lowering of the max injection pressure for the Conroy well.

Corn crop is on 3 sides with the 4th side a wood line with pine trees and mixed hard woods. There are three water wells with in 1/2 mile (2640). (1) Windon is 1240 South West of proposed Injection well. (2) Millhone is 1189 East of the proposed Injection Well. (3) Nutzling is 1550 East of the proposed Injection well. There are two intermittent streams (1) 1832 to West of proposed Injection well. (2) 2302 to East of the proposed Injection well. Production well has good identification. Well is equipped with 7 casing head, 4 1/2 casing head, with 1 1/2 tubing (valve on tubing is closed). Well has a 2 black plastic line that is connected to a 1100 gallon plastic tank (tank is empty). Well is not connected for gas production. No leaks or contamination observed during inspection. Well site has good vegetative cover established.

Halliburton R-4 (rebuilt) Packer set at 2978 with new 2 3/8 R-2 10 round tubing. Packer ran with 3 shear pins. Packer set at 25,000 lbs (tubing string weight 12,500 lbs). Packer was set at 25,000lbs. Annulus was filled with 30 barrels fresh water and 4 gallons inhibitor. Unable to test annulus due to air in the annulus.
1350555475 2012-10-17 Salt Water Injection Well No Violations Well settled down over night. Water was added on top of the packer too quickly entrapping air. This caused the well to burp air and water. The operator decided to wait until the next day to continue the mechanical integrity test on the packer. Performed three failed mechanical integrity test at the beginning of the morning, reseating the packer after each test with increasingly higher pull pressure on the drill string and packer. The fourth reseating of the packer had a total pull weight on the drill string and packer of 27000 lbs, with approximately 14000 pounds of that applied to the packer. The maximum allowable injection pressure on this well was permitted at 880 psi. This last mechanical integrity test started at 910 psi and stayed at that pressure for 15 minutes. It passed.

1527008408 2018-05-21 Salt Water Injection Well Violations Noted Follow up inspection to determine extent of out of containment contamination as a result of the facility fire on 5-20-2018. The containment dike over flowed at a low point at the south east corner of the concrete containment dike. Brine and oil over flowed the edge of containment and into a gravel and sand layer that surrounds the outside of the containment. This gravel and sand layer has a 4 inch PVC drain pipe that flows north east of the containment. Brine was observed flowing from the 4inch drain pipe to the wood line approximately 22 feet in length by a varying width from 2 feet to 7 feet. Three High Range Quantabs were used to test the flow from the 4 inch pipe. All three Quantabs maxed out at over 6620 ppm Cl. A soil sample collected in the area between the concrete dike and the earthen dike at 12 inches deep tested 2277 ppm Cl. A sample of the water standing outside the concrete dike east side maxed out at over 6620 ppm Cl. ODNR DOGRM ER Lane was called to inform him of the testing results. Jim Diddle, Spence Carpenter and Butch Barton were on site. They were instructed to remove contaminated material and fluids from the location. All fluids to be sent to an approved UIC facility and the contaminated material is to be hauled to an EPA approved landfill. A small plastic lined pit was constructed below the 4 inch PVC pipe outlet. A sump pump was placed into the pit and the water is being pumped back into concrete containment.

JD Drilling crew has excavated sand and gravel from the area between the concrete containment dike and the earthen containment dike. A field test of the soil between the containment dikes revealed 2488 ppm Cl. The discharge from the 4 inch PVC pipe is being collected in a plastic lined sump. The discharge water was tested with High Range Quantabs that revealed 776 ppm Cl. The gravel area outside of the concrete containment dike (south side) has been excavated and the water in the gravel and clay was tested with High Range Quantabs that revealed 3299 ppm Cl. ( this was after a rain event.) A JD Drilling Vac truck is on site to collect the water outside of the containment.

The last part of the clean up was a excavation that was approximately 20 feet long by 3 feet wide by 2 feet deep. The contaminated soil was removed. This satisfies the compliance notice.

The manifold was tested to the Conroy injection tubing master valve, the Showalter injection line valve on the facility side of the line, The injection line from dike A and the injection line from dike B. The line from dike B had air pockets that failed multiple tests. The valve on the manifold connecting dike B was closed and the manifold tested at 880 psi for 15 minutes with no loss in pressure. The 3 inch line connecting dike B and the manifold did not pass. JD crew is going to allow the air to work out of the 3 inch line over night. A test is scheduled for 3-21-19.
