1134139438 2014-02-25 My inspection finds the well has still not been repaired. The casing is still open to atmosphere. The well is incapable of production. The company has filled production for 2011 and 2012 even though previous inspections have determined the well was not capable of production. Photos were taken. A Chiefs Order has been prepared.
-1151881590 2014-06-16 I am onsite to follow up on Chiefs Order 2014-127. My inspection finds the well has not been repaired and the 4.5 casing is still open to atmosphere. The well is not capable of controlling the flow of gas and oil. The well is not equipped in a manner that would allow it to be produced and there are no pipelines attached to the well.
-1197945671 2014-10-10 My inspection today finds the cement top is visible at a measured depth of thirty six inches from surface. The well is completely static with no signs of flow.
1255797599 2009-10-16 Inspection revealed that well is still incapable of producing, production tank has been removed since my last inspection there is a slight odor coming from wellhead area(H2S) Sulphur odor.

1381948376 2013-10-08 My inspection today finds the 4.5 casing has failed at the surface. The casing has broken off at the top of the well head. The well is not equipped to prevent the escape of gas and oil. The casing is defective. I spoke to Charles Buckalou, well tender for Stonebridge, and made him aware of the condition of the well.
1411079326 2014-09-18 My inspection today finds the service rig crew ran 1.5 tubing to a depth of 5849'. Formation Cementing pumped 30 sacks of class A cement and displaced it with 13.7 barrels of nine pound brine.
1412950680 2014-10-09 My inspection today finds the service rig contractor, Matheny Well Service, has pulled 4362' of 4.5 casing and ran 4312' of 1.5 tubing. Formation Cementing spotted a 57 sack class A cement plug at the bottom of the tubing with 10.3 barrels of displacement. Matheny Well Service pulled the tubing up to 1714'. Formation spotted a 200' class A plug. The service company pulled the tubing out and ran wire line tools in to tag the plug and found the plug slipped down hole nearly fifty feet. The rig crew then ran tubing back down to the top of the prior plug and Formation spotted a 100' class A 1% gel plug at 15.2 pounds per gallon. The tubing was then pulled up to 200' from surface. Formation gained circulation and placed a 200' plug at the surface. The remaining tubing was pulled and the cement plug was topped of to thirty inches from ground level. The well has been plugged resolving the previous violations.
-1493074441 2013-12-12 The status of the well has not changed. The well has not been repaired and is still not capable of preventing the escape of gas or oil.
-1715626797 2014-09-15 My inspection today finds Matheny Well Service is onsite to run 1.5 tubing to the bottom of the well. Stonebridge employees are staging tanks and equipment. Formation cementing is scheduled to set a bottom hole plug on Wednesday morning.
-1751817200 2012-11-26 Stopped at the location to see if Stonebridge has moved a service rig in to begin plug back operations. The well is equipped with 4.5 casing, and is not in production.
1844639636 2014-09-26 My inspection today finds John Schneider of Stonebridge Operating has opted to perforate the 4.5 casing with one 2.125 biwire shot rather than hot tap, due to their concerns about the structural integrity of fittings on the 8.625 casing head and the inaccessibility of the 8.265 casing. After perforating 260 psi was witnessed on the 4.5 casing. Mr. Schneider decided to flare the gas to ensure the safety of personnel onsite. No H2S was recorded by any gas monitoring system.
1884876641 2006-09-22 Upon inspecting well site found well to be idle last production reported on said well was back in 1993,well is equipped 8 surface casing and 4.5 production casing with swedge and valve on top all valves are closed suspect H2S has broken in on casing, well has 1-100 bbl tank on sight that is full of holes also there is a separator on site not hooked up.
-2057855915 2014-10-06 I am onsite witness plugging operations. My inspection finds the service rig crew cut the 8.625 well head packing ring off and used a center spear to pull the casing up to a point where a coupling could be welded on. After welding on a coupler the service rig crew began pulling casing. The well is static with no gas flow. No H2S has been detected to this point.
-269068064 2007-02-15 No change in status of well (still idle venting trace amounts of H2S) have tried to contact well owner with no results left messages on John Snieders phone with no return calls noted so a Chief's Order is requested to plug/plugback said well.
-280445812 2014-01-16 My inspection today finds the 4.5 casing has still not been repaired. The well is open to atmosphere and a small amount of gas is escaping the well. I noticed a slight sulfur smell. I will contact the company about repairing the well.
325656336 2014-02-28 I called Mr. John Snyder of Stonebridge Operating at 11:17 am. I discussed the outstanding violations against the well with Mr. Snyder. Mr. Snyder stated that he was planning to get a permit to plug the well. He also stated that the proper identification would be in place by 3/7/14.

636515050 2014-10-02 My inspection today finds Appalachian Well Services ran a 10 shot collar buster to a depth of 4362'. The service rig crew was unable to break the 8.625 casing head packing ring loose to begin pulling the 4.5 casing. Work will continue tomorrow.
-800615410 2014-08-28 Attended Board of review hearing in Columbus and decision was agreed upon that Stonebridge would plug well within a sixty day time frame.
861916681 2014-02-05 Mr. John Snyder returned my call at 4:54 pm. I told Mr. Snyder that due to the fact the well has been out of compliance for eight years and the casing has broken off at the surface, the company must make repairs or plug the well immediately. He stated that he would begin the permit process to plug or plug back.
