1392735590 2014-02-12 My inspection found that Nick's Well Plugging set the bottom plug for Duck Creek Petroleum. Plug #1 was set from 4,143' to 4,489'. Nick's operator mixed and pumped 30 sacks of Class A cement through 1.5 tubing. The cement was displaced with 16 bbls of water. Next, the operator removed 4,265' of 1.5 tubing. Once the tubing was removed, Nick's operator ripped the 4.5 casing at 3,165'. Nick's crew pulled 8 joints of 4.5 casing to set Plug #2 at the Big Lime formation first thing the next day. The well was shut-in overnight.
-1902819601 2014-02-13 I witnessed Nick's Well Plugging complete the plugging for Duck Creek Petroleum. Plug #2 was set from 2,550' to 2,850'. Nick's operator established circulation with 30 barrels of water. The operator mixed and pumped 86 sacks of Class A cement. The cement was displaced with 40 barrels of water. Next, the operator pulled 57 joints of 4.5 casing. Plug #3 was set from 430' to surface. Nick's operator established circulation with 10 barrel of water. The operator mixed and pumped 131 sacks of Class A cement. I witnessed a good return of cement at surface. Nick's operator removed the final 10 joints of 4.5 casing and topped of the well with cement. After 15 minutes, the cement held static at surface. A total of 75 joints of 4.5 casing was removed from the well. All circulated water and cement was placed into a tarp covered half round container.
-769523308 2014-07-01 I was on location for a final restoration. I found during my inspection the site restored. The site was graded. Vegetation was well established. All production equipment was removed. All debris was removed. Above ground containers were used for all returns during the plugging of the well; no pits were used. A landowner waiver was received for the lease road. Restoration passed.
