1328880237 2012-01-31 On 1/31/2012, I performed a status check on the above well site. No violations were noted at this time.
1580708818 2013-03-18 On 03/17/2013 at approximately 10:05pm, I was left a voicemail message from Rusty Morrison of Atlas regarding a reported tank fire at an Atlas site on Knox-School Road. In a subsequent phone conversation at 10:25pm that evening, I discussed the situation with Rusty Morrison. It was his understanding that a 210 bbl storage tank at the Picknell-Miller Unit #1 had caught fire and exploded, blowing the top of the tank off. The local fire department had been summoned to the location, and had extinguished the fire. No oil escaped the tank. There was no oil in the dike. He indicated that all emergency personnel and company personnel were leaving the site. Mr. Morrison indicated he would be at the site at first light on 03/18/2013. At 11:08am on 03/18/2013, I had a phone conversation with Mr. Morrison. He indicated that he had been to the site. OEPA personnel had been to the site, as well as Columbiana County EMA. The current status was that the well was shut in, the equipment that had been blown off was returned to the tank battery. The tank top was retrieved and brought near the tank battery. A neighboring mobile home had experienced window damage, and Mr. Morrison arranged for repairs. Mr. Morrison indicated confirmed that there was no oil spilled, and that the remaining oil in the tank was to be hauled later in the day on 03/18/2013. A cause for the fire and explosion is still undetermined. I went to the site for a follow-up inspection, arriving at approximately 2:00pm on 03/18/2013. I found the situation at the location as Mr. Morrison had described. I walked around the tank battery and took pictures. Additionally, I went to the well head, and found the well shut in.
-1603725692 2013-04-09 I went to the site to follow up on the repairs at the tank battery following the 03/17/2013 tank explosion. I found that the damaged 210 bbl tank had been removed, and was replaced with a new 100 bbl tank. The tank had new identification labeling. The dike was in good repair following the tank replacement work. Equipment at the tank battery now includes: horizontal separator, meter box, drip bottle, concrete vault brine tank, 100 bbl storage tank, and a house gas meter assembly.
