1205655088 2015-04-15 On the day inspection accompanied ODNR Inspector Matt Fulk, I witnessed the production equipment included currently flowing with 1250psi on the tubing and 1950psi on the casing.
1359293280 2014-09-29 I walked through the production facility with Kevin Eshbaugh the Production Forman. There are three wells on pad that are producing. The reason for my visit was based on a complaint # 4818. There was no vibration coming from the pad.
1394640925 2014-03-12 On the day of my inspection, I witnessed Express Energy Services break circulation with fresh water then displace two hundred and ninety sacks of Class A cement with a weight of 15.6 and a yield of 1.18, I witnessed fourteen barrels of cement returns back to the cellar.
1398783147 2014-04-29 On the day of my site inspection I witnessed the Nomac Rig #137 tripping the drill string string in and out of the well bore to free up the tool that was hung at a depth of 12,000', the pad appeared to be stable, there was no erosion and vegetation was established.
1399985240 2014-05-09 On the day of my inspection, I witnessed O-Tex Pumping, LLC circulate the hole with fresh water then mix and displace 980 sacks of Premium H 2% cement back to surface with forty seven barrels of cement returns to surface, I also witnessed O-Tex bump the plug ,( the floats held ) before releasing the pressure.
