Well appeared to be drilled. Cuttings are to be land farmed. Site was very muddy. However, BMPs were in place and adequately controlling E&S. Brush pile was placed outside of silt fence. This should be placed inside silt fence, having the silt fence the final barrier from the site. No violations noted at this time.

Site was very muddy. BMPs were adequately controlling E&S. Pit was removed. Liner was rolled in corner of site. Spoke to Dean Walters. He stated the pit was being influenced with run-off. It was pumped/hauled and cuttings land farmed. No violations at this time.

1967084 2011-04-15 No Violations Noted Site visit was part of routine inspection.

Well appeared to be drilled and in production. Site was still very muddy. However, BMPs were still in place and adequately controlling E&S. Pit liner had begun to fall into pit. It was being held up with a tree limb jammed through the liner into the ground. Liquids were still in the pit. Adequate freeboard was available. Well was hissing loudly. Gas could be felt leaving the well head. A message was left with PA DEP inspector Jason Brannan.

Site was still very muddy. BMPs were adequately controlling E&S. Liner was rolled in corner of site and still not removed. Well head was hissing loudly; did not approach.

1970652 2011-04-19 No Violations Noted WQS MARK MAESTRA CALLED AND TOLD ME THAT THE WELL HEAD IS LEAKING. I TALKED WITH THE WELL TENDER AND HE SAID HE WOULD LET HIS SUPERVISOR KNOW ABOUT THE PROBLEM.

ANULUS CLOSED, PRODUCED FLUIDS TANK, HEATER, AND NOT RCLAIMED YET.

1971935 2011-04-19 Pending Site visit was in coordination with Jason Branan to check on leaking gas well. Leak appeared to have lessened.

Site restoration had begun. One pit had started to be removed. Liner was gone, with half of the pit filled with soil. Remaining hole was filled with water. This water was sampled 0629713 Results pending. No machinery on site. Spoke with Dean Walters concerning site. He stated restoration began but due to heavy rains work ceased. Water in pit is reported as rainwater and runoff. Machinery is being kept off site due to nearby ﾿camp﾿. A small fire ring, garbage and vehicle were there at time of inspection. Concerns of vandalism prompted the reactions of the contractor. Inspection results pending sample analysis.
