Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
2142794 Primary Facility 01/24/2013 Routine/Complete Inspection No Violations Noted Well shut in. 9 x 5 and 5 1/2 = 0 psi. 1 seperator, 1 condensate tank. Cellar filled with wter, no visible bubbling.
2144874 Primary Facility 03/01/2013 Routine/Partial Inspection No Violations Noted Conductivity sampling of the pad surface revealed elevated levels of conductivity located at the puncture location. An Extech ExStik 2 meter revealed conductivity at a level of 9.8 milliSiemens/cm3, thus confirming that fluid did indeed flow through the puncture. Further sampling revealed conductivity levels dropping as moved away from this location. SWN is preparing to install new containment for flow back procedures. Instructed SWN to vacuum up the free standing water near and around the puncture location. Go ahead and place new containment down. Complete flow back procedures. Right after flow back procedures complete, remove containment, survey pad and identify the extent of the contamination, remove contaminated soil, conduct confirmatory sampling, etc. End result of clean up should at a minimum meet Act 2 requirements. Contact Department when excavation and sampling is to occur. Violations associated with this site being handled under inspection ID 2144808.

2656370 Primary Facility 2017-10-30 Routine/Complete Inspection No Violations Noted On October 30, 2017, the Department conducted a routine inspection at the SWN Production Company, LLC, GU-Clark-Jennings-7-Pad in Herrick Township, Bradford County. Andrew Hobbins represented the Department. Nobody was present for SWN.

The site sign, E & S plan and weekly inspection logs were on site and available for review. The site was well vegetated and no erosion or sedimentation issues were observed. The well pad has not been reduced in size, and PCSM has not been addressed at the site. The Department recommends following the stipulations of the Consent Order & Agreement regarding site restoration.

Three wells were in production during the inspection. The wells were labeled and no bubbling was observed. One production tank and three GPUs were on site. All conductivity readings taken from puddles on the pad were low in specific conductivity. No staining was observed on the surface of the well pad.

No violations were observed during the inspection.

2720336 Primary Facility 2018-04-19 Routine/Complete Inspection No Violations Noted Three wells staged for production. No leaks detected. Site stable. No violations noted.
2816555 Primary Facility 2018-12-17 Site Restoration No Violations Noted The DEP conducted a RESTORATION inspection of SWN's Chilson Jennings (7 PAD) in Herrick Twp, Bradford County on 12/17/2018 at 1130 hours. The weather was cloudy and 37 degrees. The inspection was conducted in response to a well site restoration report received by the DEP.

This site currently has three wells in production with one production tank. The large portion to the west of the pad that includes the infiltration berm has recently been disturbed. The area is covered with hay/straw but 70% perennial vegetation has not been achieved. Also, temporary E&S BMPs (silt sock) was still in place to prevent erosional discharges. There is also a smaller portion along the access road with similar characteristics.

This well site restoration report is considered returned at this time. Upon permanent stabilization and removal of temporary E&S BMPS and resubmission, the report could then be accepted.

No violations noted today, I departed the site at 1200 hours.
