Dept requested a follow-up phone call.No follow-up call received.No E&S issues observed at site or AR.This insp is PENDING. Viols may be issued based on the investigational findings of follow-up insp.UPDATE: Follow up Insp.At time of insp WT present but with incorrect Opr information. Crack still visible on BT. No active leak observed. Dept contacted Opr Rep Jake Stilley to recommend updating WT and monitoring BT to ensure structural integrity is not compromised. Mr. Stilley stated that he would send the well tender out today.On 12-30-15 Opr contacted Dept stating well tag has been replaced and BT is being removed because well no longer produces production fluids.This insp remains PENDING. A follow up insp will be conducted. |
