0589072137 2009-06-19 Production Wells No Violations The two siphon dams can be removed and the entire affected area can be reclaimed. The well still cannot be turned back into line until the containment dike is repaired.
0923363801 2009-06-16 Production Wells No Violations No change in the status of the well and tank battery. The lower siphon dam can be removed and the area reclaimed. The booms can be removed from the retention pond. The secondary containment dike still needs to be rebuilt before the well can be turned on and the upper siphon dam can be monitored for the next week.
1155309041 2009-06-03 Production Wells No Violations I received a call from Rick Batta at North American, reporting an oil spill at the Novisky well. The spill is a rule violation, but the company response was reasonably quick and effective. If the subsequent actions continue in a timely manor, a NOV will not be issued. The spill was caused by two events; 1) overfilling the 140 bbl stock tank and 2) insufficient secondary containment capacity. The spill exited the containment area and ran down the lease road to the landowners driveway, down both sides of the driveway via drainage ditches and into a retention pond. The driveway, ditches, culverts and pond were all impacted. According to the landowner, there are no discharge points from the retention pond other than overflow. The current pond level is at least three feet below that spill point. Moore Well Service was on site when I arrived and had started recovering oil with a vac truck. Additional equipment was enroute to install two siphon dams and continue product recovery. Additional recovery will continue tomorrow with hot water flushing and oil recovery.
1194542623 2009-06-26 Production Wells No Violations The affected areas have been graded but still need to be seeded and mulched. The siphon dams have been removed. The secondary containment dike has been raised several inches but needs additional attention. I passed this information on to Rick Batta.
1292102991 2009-07-09 Production Wells No Violations All of the spill issues have been addressed and corrected. I called Rick Batta and gave him permission to turn the well back into production.
1502102710 2009-06-04 Production Wells No Violations Two Moore Well Service vac trucks were on site flushing the impacted areas, when I arrived. A steam truck would not be available until tomorrow. The plan for today calls for flushing the impacted areas with fresh water and recovering any free oil. The catails in the pond will be cut off to allow for skimming the pond with both booms and suction.
1691679368 2009-06-05 Production Wells No Violations The hot water treatment was completed before I arrived on location. The majority of the spilled oil has been recovered from either the siphon dams or the retention pond. The siphon dams will remain for the next several weeks to collect any residual product and will be checked on a regular basis. I advised Rick Batta in a phone message, that the well cannot be turned back on until the secondary containment system meets the SPCC requirements.
2024730297 2009-06-11 Production Wells No Violations The lower siphon dam and the retention pond are in good condition. The lower siphon dam can be removed and the area can be graded and seeded. Any remaining impacted vegetation around this siphon dam can be removed. The upper siphon dam is still collecting small amounts of oil being released from the lease road stone, and needs to remain for the near future. The secondary containment dike around the tank battery still needs to be rebuilt before the well can be turned back on.
