0569648348 2012-08-29 Preliminary Restoration No Violations Met with Mr. Mike Reeves from CEC (Civil and Environmental Contractors). They are a third party contractor for PDC Energy and will be making recommendations for the completion of the preliminary restoration on this well site. We inspected the site together and came to a consensus on how to remedy the erosion problems.
1190763327 2019-02-07 Production Wells No Violations My inspection of the well revealed that it was shut in and not capable of production. The well was equipped with a 4.5 inch to 2 inch swage with a gate valve. I did not observe any production lines connected to the well. According to production records the well has never produced any gas or oil. I took several photos of the well location and lease road. I left a voicemail message for Doug Kitchen at 2:16 PM on 2/8/2019. The well owner/operator shall submit a written plugging schedule to Brandon Mitchell at 5880 Memory Road Zanesville, Ohio 43701-9553 within 30 days, per ORC 1509:13(B)(7), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied. Time of inspection: 10:30 AM. Weather: 56 degrees F, Rain
1315502989 2011-08-29 Drill / Deepen / Reopen No Violations Witnessed cementing of 7 intermediate string. Good circulation was achieved.
1348228477 2012-09-18 Preliminary Restoration No Violations Met with 2 company representatives from CEC. They wanted to assess where the lease road crosses the stream. We also looked at the drainage from the rest of the lease road.
1409056112 2014-08-26 Production Wells No Violations I received a request from Doug Kitchen to confirm the clean up of a brine leak from a tank that was being used to hold brine. The property is owned by Mr. Greg Lint. I received a conference call with Mr. Kitchen and Mr. Lint. I told both partys I would conduct a investigation to determine if the spill area was cleaned up properly. I obtained three soil samples to check for chlorides. I added water to make a slurry to determine if any chlorides were present. The samples were taken 20 feet, 30 feet, and 40 feet from the tank that leaked. The sample locations were down hill were the brine would have flowed. The highest reading was 106 PPM using low range Quantab Titrators. I called Mr. Lint back to inform him of my findings.

1647714561 2019-02-07 Preliminary Restoration No Violations I conducted a preliminary restoration status check on this well today. During the inspection I observed the well location for vegetation. My inspection revealed that vegetation was sparse. I observed substantial erosional ruts on the lease road. I took several photos of the well location and lease road. The well owner shall establish vegetation at the well location and correct rain water runoff along the lease road to prevent erosion. 1820496031 2012-08-15 Administrative Inspection No Violations Spoke with Jeff Glossa from PDC at 3:29 pm. We spoke about the preliminary restoration on this well and another (p# 3601). I told him that it was past due and needed to be completed as soon as possible. I also made mention of the lack of vegetation and erosion problems. He stated that PDCs environmental compliance person had been on scene and was evaluating the situation. He then stated that he would have Randal Ferguson call me. He also works with their environmental department. Mr. Glossa also reported that he had been in contact with Columbus as far as the completion of the well and had submitted all necessary paperwork.
