0206885553 2017-08-21 Completion Testing No Violations On the date of my inspection I found a work over rig from Stone Well Service rigged up on permit 34-081-2-0675 attempting to re-establish the ability to pump down hole by drilling out 2 stage plugs. Halliburton is also on site with a frac crew waiting for the work over rig to perform the maintenance on the wells so they can continue with frac operations. All of the wells on the pad require some maintenance before frac operations can resume. This well has been hydraulically fractured through stage 22 of 96 stages. Chesapeake anticipates the frac operations to be able to resume by 8/24 or 8/25.
0508973447 2017-08-10 Completion Testing No Violations On the date of my inspection I found a Halliburton frac crew rigged up on all 4 wells on the pad. While I was on site I witnessed Halliburton pump the frac job for stage 5 of 96 for this well. All hazardous materials are within double containment which is in good condition, free of visible punctures or tears. The location is surrounded by a fenced berm with healthy vegetation. The pad drain valves show they are in the closed position.

1211959154 2017-09-19 Completion Testing No Violations On the date of my inspection I found a Halliburton crew rigged up on all 4 wells on the pad for hydraulic fracturing operations. While I was on site I witnessed Halliburton pump the frac job for stage 46 of 52 on permit 3408120675 and stage 40 of 46 on permit 3408120671. They are currently on stage 87 of 96 on this well. All hazardous materials are within double containment which is in good condition, free of visible punctures or tears. The location is surrounded by a fenced berm with healthy vegetation. The pad drain valves show they are in the closed position.
1498134051 2017-06-22 Casing String Witnessed No Violations I am on site to witness the cementing of 13.375 inch surface casing. Nomac 71 drilled a 17.5 inch diameter hole to a depth of 588 feet and set 578 feet of 13.375 inch 54.5 pound per foot casing. Schlumberger established circulation with fresh water, pumped 505 sacks of class A cement, 1.2 yield and 15.6 pound per gallon surface set cement. Approximately 20 barrels of cement returned to surface. The wiper plug landed and pressure was increased to approximately 500 pounds per square inch (psi) over pump pressure, the casing held approximately 812 psi for 5 minutes.
1536801721 2017-04-08 Casing String Witnessed No Violations I am on site today to witness the cementing of 20 inch conductor casing. Express Energy Services drilled a 24 inch diameter hole to a depth of 120 feet and ran 120 feet of 20 inch diameter 94 pound per foot conductor casing. Express 8 barrel of water ahead to verify circulation then pumped 230 sacks of class A cement, 1.18 yield, 15.6 pounds per gallon with 2 percent Calcium Chloride and 0.5 pound per sack flow seal. I witnessed good cement returns to surface.
