1954312 Primary Facility 2011-03-01 Drilling/Alteration No Violations Noted atteempt at nitro foam frac, created pressures too high during treatment to remain within safe paramiters, upto 9000# , shut down nitro and will return at later date with conventional hydro-frac, treated broke stage with sand an water before leaving
1963890 Primary Facility 2011-04-06 Drilling/Alteration No Violations Noted well shut in after nitro frac need to have gauge on 9 5/8' x 5 1/2' annulus

2217886 Primary Facility 2013-10-29 Incident- Response to Accident or Event Violation(s) Noted Flowback discovered under liner on 10/24/13. Called in on 10/25/13. Volume uncertain. Viols closed 3/24/2014 with approval of Remedial Activity Report
2227053 Primary Facility 2013-11-22 Incident- Response to Accident or Event Outstanding Violations - No Viols Req'd Gas buster not yet moved so pad beneath not evaluated, Elevated conductivity puddle adjacent.
2255163 Primary Facility 2014-03-19 Routine/Complete Inspection No Violations Noted
2481080 Primary Facility 2016-05-04 Routine/Complete Inspection No Violations Noted The site currently has >70% coverage of perennial vegetation and temporary erosion and sediment control BMPs have been removed. No evidence of accelerated erosion or sedimentation was observed.

The electrical conductance of puddles was not screened due to the absence of puddles.

Because the site is under 5 acres in size, no Erosion and Sediment Control General Permit exists for this site. No current well permits exist. Chief has indicated that it plans to partially restore the well pad by spring 2019. In order to remain in compliance with the Department's regulations regarding site restoration (Title 58 Pa. C.S. § 3216), Chief should obtain a new drilling permit or request a 2-year extension of time for site restoration.

No violations were cited during this inspection.

2562284 Primary Facility 2017-02-16 Routine/Complete Inspection No Violations Noted No violations cited.
2589676 Primary Facility 2017-05-03 Routine/Complete Inspection No Violations Noted One well in production phase. One separator within containment. One suction engine on location. No leaks detected from production equipment. Site stabile. No violations.
2650789 Primary Facility 2017-10-23 Routine/Complete Inspection No Violations Noted Arrived at 14:34. Weather: overcast. 1 producing well onsite. Cellar gravel-filled. One annular vent, open plumbed to 550-galllon polytank (no combustible gas detected at tank vent). Well on compression with artifical lift. 350 psi(0-3000psi analog) at top gauge.
2784680 Primary Facility 2018-10-03 Routine/Complete Inspection No Violations Noted Arrived onsite for a routine inspection. Previous rainfall formed a few puddles on the pad. One of these puddles had a conductivity of 138.5 µS/cm.

There is 1 properly tagged Marcellus well staged for production. There is 1 GPU, 1 compressor, and 2 produced water tanks onsite and in secondary containment. Water in GPU and produced water tank secondary containment had conductivities of 43.8 µS/cm and 119.4 µS/cm respectively.

The pad has been reduced in size and is well vegetated. PCSM BMPs were present and appeared to be functioning as designed. E&S inspections are being conducted and the log was present onsite. All other pertinent paperwork was also available in the mailbox.

No violations noted.
