The rig was on the 4H at a depth of 75'. The 2H was completed at a depth of 180'. The 3H was completed at a depth of 176'. The 5H was completed at a depth of 175'. They plan to drill the 1H when the 4H is completed. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted. |

At time of inspection. I met with Dillon Otten of SWN. We noticed slight bubbling at the (13x9) annulus. Dillon showed me a video of the bubbling taken on 6/8/18, the bubbling was steady in the video and appeared to have diminished by the time of my inspection. We filled the annular space with water to the top of the casing. There was no gas detected with my Altair 5X gas meter, last calibration was 6/7/18. I suggested SWN monitor the well and contact the Department if the bubbling increases and Dillon agreed. There were no violations noted. |

There were 15 stages completed out of 24 on the 2H. There were 18 stages completed out of 26 on the 3H. There were 15 stages completed out of 20 on the 4H. The water was being pumped to the location from the Zeffer production fluid impoundment and the Young fresh water impoundment. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted. |

We talked to Donnie Torres (Well Site Supervisor-SWN). He informed us that crews were setting up on site to perform drill out and flowback operations. The one producing well on the pad was shut in. The others have been fully drilled and stimulated. Drill out operations on the plugs had not yet began. The release occurred on the southwest corner of the pad, when a blender and its associated piping failed, causing a release of production fluid and freshwater in containment. The estimated release volume is not known. Material in the immediate area under the containment was screening high for conductivity. The material was excavated, but more high-conductivity water kept infiltrating the sump area below the containment. The well was recently completed and put in production. The well tag was on the well and correct. The cellar was set up with a plastic corrugated pipe around the well for inspection access. There was water .5' below grade in the cellar and there was no bubbling noticed. There were twenty 500bbl frac tanks on containment from a recent frac. Most of the tanks were empty and the containment appeared to be in good condition. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted. |
