2435592 Primary Facility 2015-12-22 Routine/Partial Inspection No Violations Noted On Tuesday December 22nd, 2015, I conducted a routine/partial inspection of the Fowler Well Site located in North Bethlehem Township, Washington County with John Parry (DEP) following an operator requested site meeting concerning potential modifications to the current permit. The following observations were made:
2440786 Primary Facility 2015-12-31 Follow-up Inspection No Violations Noted On 12/31/15, WQS Supervisor John Carson, Environmental Trainee John Parry, and I performed a follow up inspection at the Fowler Well Site in response to questions raised by the operator in regards to a 12/22/15 violation at the same site and made the following observations:

KOP was at 7196 ft. EOB on the curve was at 8229 ft. MD (7554 ft. TVD). TD was at 15,898 ft. MD (7527 ft. TVD) on 1/25/16. The 5 ½” casing was set at 15,890 ft. and cemented to surface with 10 bbls returned.

2473589 Primary Facility 2016-03-31 Administrative/File Review In Compliance with Policy An equipment malfunction caused some production water to be released onto containment. The containment had previously been vacuumed on March 30, 2016 due to recent heavy precipitation. The remaining rain water mixed with the production water within containment. On March 31, 2016, some heavy equipment inadvertently ran over the berm of the containment, thereby damaging it and releasing the estimated five gallon mixture of rain water and production water from containment and onto the pad. Their incident response team was notified and responded by vacuuming up the fluids and removing the impacted material for disposal. It was later estimated that three to four gallons of the mixed fluid had been released.

No impact to water had occurred. The incident was promptly reported to the Department and the area remediated. The incident has been determined to be In Compliance with Policy (ICP). No violations are being cited at this time.

2536498 Primary Facility 2016-03-08 Compliance Evaluation No Violations Noted On 3/8/16, I performed a follow up inspection at the Fowler site accompanied by DEP engineer Emmanuel Nwaobilo in response to previous violations noted on 12/22/15 and 12/31/15 and made the following observations:

•Site plans showing the updated access road were not present on site at the time of inspection. No violation, remediation requested. •A drainage culvert on the south side of the access road was poorly constructed and at the time of inspection allowing sediment laden water to leave the site. Remediation recommended, no violation.
