0326271765 2016-10-04 Production Wells No Violations I conducted a status check inspection on this well. The well is to be plugged the week of 10-10-2016. The pumpjack has been removed. The rods and tubing are still in the well. The tank battery reclamation is almost complete. Don Nething (Ohio Valley Energy) will be the Field contact.
0565199799 2018-03-07 Final Restoration No Violations I did a Final Restoration inspection for this well. The site is graded with no active erosion and vegetated. A landowner waiver was received by the Uniontown Office of ODNR Oil & Gas on 11-03-2016 waiving the removal of the buried lines. The final restoration is passed.
0622374576 2015-09-16 Preliminary Restoration No Violations I was on location to check this well for preliminary restoration. The electrically driven pumpjack is currently shut in, contains a house gas connection and is fenced and landscaped. The well contains identification and has all required locks and valves. The tank battery is fenced and landscaped with identification. The tank battery contains a 140 bbl production tank, 100 bbl brine tank, a dike, mechanical separator and an identification sign. Vegetation on location has been established at this time. The identification signs posted on the wellhead and tank battery do not contain the address of the well and other required information. I have contacted the operator and new identification signs with the required information will be posted at the wellsite this week. I will perform a follow up inspection to ensure the proper identification is posted and will pass preliminary restoration at that time.
1476383650 2016-10-04 Production Wells No Violations I conducted a status check inspection on this well. The well is being prepared to be plugged and abandoned. The pumpjack has been removed as well as the tank battery. The rods and tubing are still in the well and a 100 barrel tank is still on location.
1480531084 2016-11-28 Final Restoration No Violations I conducted a final restoration inspection on this well. The original signature landowner waiver was received at the Uniontown office of Oil & Gas on 11-03-2016. The well and tank battery locations have been graded, seeded and mulched. There is no active erosion and the final restoration can be passed once vegetation has established.
1494602274 2017-05-12 Final Restoration No Violations I conducted a Final Restoration inspection on this well. The final restoration cannot be passed at this time. The well and tank battery site lack vegetation and both areas are ponding water. The area around the restoration is lawn which is being maintained as such. I left a message for Don Nething (Ohio Valley Energy) and advised him the final restoration cannot be passed at this time.
