0204802170 2018-01-02 Production Wells No Violations On the day of my inspection following up on Compliance Notice 1697061644, I witnessed there was no change in the status of the well since the inspection on 11/27/17. The well was equipped with 4.5 inch diameter casing, 2.375 inch diameter tubing, rods, an Emsco pumping unit powered by electricity and domestic gas service with 1 of 2 valves open on the casing supplying the domestic gas. Also present plumbed from the tubing, a 225 gallon plastic tank with 185 gallons of produced fluids in it near the wellhead with no dike. Contamination was found in a 5 feet radius around the wellhead and near the drain valve on the plastic tank. No identification was present and production records were current. I placed call to Patchwork Oil and Gas and spoke to Jessica Lott who stated they never received the notice but she did have a non-receipt of a certified letter from the Division. I stated what needed done to bring the well into compliance.

1297854619 2018-03-26 Production Wells No Violations On the day of my inspection accompanied by ODNR Inspector Jeff Adams and following up on Compliance Notice 1697061644, I witnessed there was no change in the status of the well since the inspection on 2/7/18. The well was equipped with 4.5 inch diameter casing, 2.375 inch diameter tubing, rods, an Emsco pumping unit powered by electricity and domestic gas service with 1 of 2 valves open on the casing supplying the domestic gas. Also present plumbed from the tubing, a 225 gallon plastic tank with 185 gallons of produced fluids in it near the wellhead with no dike. Contamination remains in a 5 feet radius around the wellhead and near the drain valve on the plastic tank. No identification was present and production records were current.
1322776851 2018-10-25 Production Wells No Violations The purpose of my inspection was to follow up on Compliance Notice numbered 1697061644. The compliance notice stated that the tank was in violation of spacing requirements. During my inspection today, the tank remained within the maximum allowable distance from the wellhead.
1527779509 2018-05-31 Production Wells No Violations At 7:19 p.m. 5/30/18 Columbia Gas called and stated that they were at a residence at 65045 Pigeon Gap Road Cambridge Ohio and they have located multiple leaks in a gas line coming from a well. During my inspection on 5/31/18 I found 5 spots that appear to be leaking within a natural gas domestic gas line that was traveling from the house to the well owned by Patchwork Oil and Gas. I spoke with Roger Whited the owner of Patchwork Oil and Gas and Mr. Whited stated he would have someone out to shut the domestic gas service off to allow time for the line to be repaired. A Patchwork representative arrived and shut the valve off on the 4.5 inch diameter production casing that was supplying the domestic gas. The lines were no longer leaking. All valves at the wellhead were in the closed position. While discussing the issue with Roger Whited I brought up the fact that the well remains in violation of 1501:9-1-07(A)10, 1501:9-9-05(A)2, and 1501:9-9-05(A)9. The violations were written in the compliance notice 1697061644 that was issued to Roger Whited. Mr. Whited stated that he would be out as soon as he found the time to correct the violations.
1530894892 2018-07-05 Production Wells No Violations The purpose of my inspection was to follow up on Compliance Notice numbered 1697061644. The compliance notice was issued to Roger Whited for violation of tank spacing requirements. During my inspection today, the tank remained within the maximum allowable distance from the wellhead. I will continue to contact Roger Whited and work with him to correct the violation.

Contamination was found in a 5 feet radius around the wellhead and near the drain valve on the plastic tank. No identification was present and production records were current. I received a call from Roger Whited on 1/10/18 stating that he would address the issues.

1697061644 2017-11-27 Production Wells Violations Noted On the day of my inspection I witnessed the well was equipped with 4.5 inch diameter casing, 2.375 inch diameter tubing, rods, an Emsco pumping unit powered by electricity and domestic gas service with 1 of 2 valves open on the casing supplying the domestic gas. Also present plumbed from the tubing, a 225 gallon plastic tank with 185 gallons of produced fluids in it near the wellhead with no dike. Contamination was found in a 5 feet radius around the wellhead and near the drain valve on the plastic tank. No identification was present and production records were current. Owner/Operator shall properly identify well in accordance with OAC 1501:9-9-05 (A) (10). Owner/Operator shall meet proper setback distance from the well to the 225 gallon tank on location in accordance with OAC 1501:9-9-05 (A) (2). Owner/Operator shall remediate affected areas by removing soils, replacing with clean fill and disposing of properly.

1986787432 2019-01-18 Production Wells No Violations The purpose of my inspection was to follow up on Compliance Notice numbered 1697061644. During my inspection the well was equipped with 4.5-inch diameter production casing, 2.375-inch diameter tubing, rods, a pumping unit. Production valves were in the open position. A domestic gas line was attached to the production casing with an open valve. The contamination at the wellhead that resulted in compliance notice 1697061644 has now been remediated resolving the notice. While burying a new domestic gas line up to the 4.5-inch diameter production casing the contamination was remediated. The production equipment was located on the opposite side of Pigeon Gap road and consisted of one 210-barrel tank, one compressor, and one chart meter with current chart. The identification was located on the 210-barrel tank.
