Inspection Data

Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
1843296 Primary Facility 2009-11-05 Routine/Complete Inspection No Violations Noted Drill rig located onsite and drilling was ongoing at the time of the inspection. Below grade pit located onsite and it had more than two feet of lined freeboard. No accelerated erosion & sedimentation was observed in relation to the access road or well-site. Noted that the culvert outlets were rocked properly, however they outlet into a disturbed area, thus negating the purpose of the sediment removal erosion controls. The Department recommends that the operator mulch the exposed disturbed area, especially the area below the access road where the culverts outlet.
1850004 Primary Facility 2009-12-15 Routine/Complete Inspection No Violations Noted Drilling had been completed, site had been partially regraded and the below grade pit had been backfilled. No accelerated erosion & sedimentation was observed in relation to the access road or well-site. Noted that the culvert outlets were rocked properly, however they outlet into a disturbed area, thus negating the purpose of the sediment removal erosion controls. The Department recommends that the operator mulch the exposed disturbed area, especially the area below the access road where the culverts outlet, as stated in the previous inspection dated 11/05/09.
1884106 Primary Facility 2010-05-13 Routine/Complete Inspection No Violations Noted Well appeared to beequipped for and in production at the time of the inspection. Brine tank located onsite, and it appeared to be holding fluids but a large dent was observed in the tank. The site had been regraded, seeded & mulched and it was noted that the vegetation had begun to grow however it did not meet the 70% uniform coverage criteria as of yet. The operator should continue to monitor the site to insure that the 70% uniform vegetative cover criteria is met.
