0031782615 2015-06-22 Production Wells No Violations Upon inspection, it was observed that this well is currently producing. The well is equipped with a standard production tree and is producing on the 5.5 production casing as well as the 2.875 tubing. Gauges on the well read 366 psi on the tubing and 507 psi on the casing. The tank battery consists of 6 400 bbl production tanks and 3 large mechanical separators. There is also 3 flow control separators on location as well as a large degassing tower. There is a metal, poly lined containment dike around the tank battery. Legible and correct ID information is present on the tanks as well as the entry to the lease road. No violations were observed.
0384235053 2013-05-29 Drill / Deepen / Reopen No Violations Total depth at the time of inspection was 11559 ft. The inclination was 89.66 degrees. The azimuth was 1.94 radians (349.2 degrees). Spoke with consultant on site.
0392223383 2014-07-14 Preliminary Restoration No Violations Upon inspection, it was observed that a vegetative cover has been established throughout the limits of disturbance for this pad location. Any and all drilling equipment has been removed from the location, and any and all pits used for drilling activities have been filled. This satisfies the requirements for the preliminary restoration. Accompanied on this inspection by DOGRM Engineer Jason Simmerman, and DOGRM Engineering Technicians Andy Thomas and Anthony Carson.

0433926896 2017-11-27 Production Wells No Violations The inspection this day found that the well is producing using a plunger lift system. The valves are open on the wellhead and gas could be heard flowing through the well at the time of inspection. The tank battery for this well is equipped with six (6) 410 barrel tanks with identification posted near the tank battery. There are no issues to report and the well is in production.

0950932751 2013-05-31 Drill / Deepen / Reopen No Violations Received phone call from Mr. Larry Howell at 11:20 pm and he notified me of a drilling mud spill that came out of the flare line. When I arrived on site, crews were already cleaning up the mess. The majority of the drilling mud collected on the drilling pad in the stone. There is a plastic liner under the stone. All contaminated stone was collected and put in a dumpster of residual waste, and hauled to an EPA approved landfill. There was a mist of drilling mud that sprayed the grass outside of the drilling pad in an area 25 ft. long by 15 ft. wide. I spoke with Mr. Vaughn Staple about remedial work on this area. I gave him the options of either removing all contaminated materials and replacing them with clean materials or bio-remediation in place which means tilling the area up and adding lime and fertilizer or manure and working that into the soil. I checked with the consulting mud engineer and the drilling mud is 100% bio-degradable.

Fresh soil had been put in its place. Spoke with consultant on site. He also gave me paperwork for the 5.5 in. casing that they had ran and cemented.

Spoke with company man from PDC and from Spartan coiled tubing services. They showed us the tools they will be using and gave us a run-down of how the job will be conducted.

1411053385 2013-06-11 Drill / Deepen / Reopen No Violations I checked the remediated area outside of the drilling pad. It had been seeded and strawed. A crew was there smoothing the stone on the pad out. Well had a new well head installed on it.
1559913119 2013-04-15 Drill / Deepen / Reopen No Violations Witnessed the topping off of the conductor cement job. Conductor hole drilling crew was de-mobilizing and getting off of location very quickly after that.
1609620486 2013-12-13 Administrative Inspection No Violations Received phone call from Mr. Tony Long from PDC energy at 9:57 am. He stated that an oil trucker didnt get one of the valves completely shut on one of the production tanks and it released 10-15 barrels of oil into the containment around the production tanks. Their containment dike is lined and no oil escaped it. They will have it sucked out as soon as possible. I recommended him calling the EPA and notifying them of the spill but also making sure to tell them that is was completely contained.
1622109285 2013-04-11 Drill / Deepen / Reopen No Violations Met with Mr. Don Kreager at the site. He was waiting on surveyers to plot where the well stake went. They were also in the process of getting the conductor rig ready to drill in the conductor.
1665097544 2013-07-31 Drill / Deepen / Reopen No Violations Checked on site to see how close they were fracing. Pump trucks, blenders, and support vehicles were in place. Mega tanks were being filled with water and several 500 barrel tanks were being filled. Spoke with Don Kreager via cell phone and he stated that they were going to try and frac the next day.
1773851228 2013-05-31 Administrative Inspection No Violations I received a phone call at 12:55 pm from Randall Ferguson from PDCs Denver office in regards to the spill from the night before. He stated that there was a 10 barrel loss and that they removed all contaminated soil, disposed of it properly and replaced it with fresh soil.
2024051832 2013-06-17 Drill / Deepen / Reopen No Violations Company was performing pressure test on 5.5 in. casing. They pressured up to 5000 psi. Spoke with Mr. Blake Roush on site. I also looked at the above ground containment that will be used to store fluid for well stimulation.
