0661157698 2015-11-02 Final Restoration No Violations I was on location to inspect the Final Restoration status of this recently plugged Domestic Well. The location has been graded and the area is being incorporated into the agricultural field. The production equipment was removed years ago and the lease road is the landowners access to the field. Two (2) joints of 4.5 production casing were left, at the request of the landowner, for his use and remain on location. Final Restoration is approved. No violations.
1447165825 2015-10-26 Plug / Plug Back No Violations I was on location with Nicks Well Plugging to witness the setting of the bottom plug in this Clinton Sandstone well. Nicks spotted 4.7lb/ft., 2.375 tubing at a depth of 4,300 (the hole was filled-up 10 below the bottom perforation of 4,299). Nicks rigged up to the tubing, established circulation, and then mixed and pumped 6.3 barrels of cement slurry. They displaced the slurry to a depth of 3,944 with 15.2 barrels of water and pulled the tubing from the hole. The wellhead was packed in and operations were shut down until 10/27/15. See plugging report for more details on this job. No violations.
1872244818 2015-10-27 Plug / Plug Back No Violations I was on location with Nicks Well Plugging to witness plugging operations. Nicks ran in the hole with the knife tool and ripped the 4.5 production casing at a depth of 2773. Plugs were set at the top of the Big Lime, across the Berea Sandstone-Ohio Shale interval, and across the Glacial Deposits. Good circulation was established prior to setting each plug, and good cement circulated to surface on the final plug. The hole was topped off with 5 sacks of cement and the cement held at the top of the hole. See plugging report for more details on this job. No violations.
