2352363 Primary Facility 2015-03-13 Complaint Inspection No Violations Noted On 3/13/2015, the complaint contacted the Department concerning the odor allegedly coming from the Rhodes well pad. Ms. Beacker and I arrived onsite at 10:00 and spoke with the well tender (John Moltz). The well tender, Ms. Beacker, and I walked the site. The production equipment all appeared to be functioning properly but an odor similar to what the complainant described was evident. The well tender noted that the odor was a normal part of the operation of the well and specifically the dehydration unit. Approximately 10-20 barrels of spent glycol (Triethylene glycol) was being stored onsite in containment. The odor described by the complainant and noted by the Department is described as an odor but is not characterized as a mal-odor. As discussed on 3/16/2015, please review the O&M procedures for the dehydration unit and makes sure the unit is functioning properly. No Violations Noted.
2419801 Primary Facility 2015-10-27 Routine/Complete Inspection No Violations Noted At the time of inspection: arrived @ 15:26. Three wells were spudded, cellars were dry. Annuli were plumbed into production tanks. Also onsite were 2 production tanks, 3 production units, and a sand separator. A technician was calibrating gauges on the production units at the time of my inspection. No violations were noted during the inspection. Left location @ 15:35.

2454130 Primary Facility 2016-02-25 Routine/Complete Inspection No Violations Noted At the time of the Well Site Restoration Report inspection, three wells onsite are in production.

All permanent PCSM control features identified in the permitted plans have been installed. But, the infiltration basin slopes lack vegetative cover and have noticeable erosion. Silt sock around the site was cut open but the synthetic fabric has not been removed. Silt sock above the access road was left intact and was not removed. Plastic netting from erosion control blanketing was left at several locations. On the fill slope at the northwest side of the pad, the slopes show signs of recent (the last 24 hours) and historic erosion. Overall, the site lack 70% uniform perennial vegetative cover, shows signs of accelerated erosion, and has plastic material from erosion control blanketing and silt sock that have not been removed. I recommend the Well Site Restoration Report be denied.

2537913 Primary Facility 2016-11-14 Routine/Complete Inspection No Violations Noted Arrived @ 12:30, weather sunny, warm. All three wells onsite were in production. Annuli were plumbed into production tanks. Cellars were filled with gravel. Met with lease operator Jon Moltz. (all psi readings digital).

Rhodes 1H: tubing: 1223.7 psi, casing: 1130.8 psi Rhodes 2H: tubing 1217 psi, casing: 1021 psi Rhodes 3H: tubing 1507.9 psi, casing: 1139.3 psi. A slope slip was evident near the entrance gate. Moltz said that Atlas was aware of this and planning to fix it. Confirmed this with WQS on return to the office. No violations were noted during the inspection. Left location @ 13:00.

2697894 Primary Facility 2018-02-21 Routine/Complete Inspection No Violations Noted Weather was 53° and cloudy at time of inspection. Met with Atlas representative John Moltz on location. Well had 3 annulus vents plumbed to surface with analog pressure gauges on them. All 3 gauges read 0 psi at time of inspection. Well had a digital pressure gauge attached to the tubing and the production casing. The casing pressure was 1263 psi and the tubing pressure was 969 psi. No violations were noted at time of inspection.
2782567 Primary Facility 2018-10-02 Routine/Complete Inspection No Violations Noted Three wells stage for production. Three water separators. One sand separator. No leaks detected from production equipment. No violations noted.
2809717 Primary Facility 2018-12-04 Routine/Complete Inspection No Violations Noted Weather was 34° and cloudy at time of inspection. There were no Diversified Oil and Gas representatives on location at time of inspection. Well hard digital pressure monitoring equipment attached to the production casing in the production tubing. Production casing was 1145 psi and production tubing was 1141 psi at time of inspection. There were 3 annulus vents plumbed to surface at time of inspection. All 3 vents were plumbed together and ran to the production tanks. No violations were noted at time of inspection.
