0084342094 2017-05-03 Orphan No Violations On location for the Brenneman Orphan Well Emergency Plugging Project by Hydrocarbon Well Service. The rig mats requested through Field Order #1 and Change Order #1 arrived on location. These mats were set from south to north over an untiled section of the field. After these mats were placed the rig was brought back to the well. The remaining equipment will be brought back to the well tomorrow, May 4th. Terry Whitehair from the Division of Oil & Gas Resources Management Emergency Response Section was on location to flip or replace the booms placed by Ohio EPA and Interdyne back on April 8th.
0265330097 2017-04-11 Orphan No Violations On April 11th, 2017, the fluid level was at approximately ground level. The exposed sections of tile in the field still have a sheen that is diminishing, but it is unclear if the riser is containing the oil or still allowing for contamination to

the drainage tile. This well will be referred to the Orphan Well Program.

0280258707 2017-06-26 Orphan No Violations On location of the Brenneman Orphan Well by Hydrocarbon Well Service. On Sunday June 25th, the plug set on Saturday June 24th had settled to 50 feet in the 7 inch casing and to 15 feet in the annular space. The remaining space was filled with 11 sacks of cement. On Monday June 26th, I was on location with Rob Leidy and Paul Corder of the Division of Oil & Gas Resources Management Radiological Safety Section for the testing of all casings and tubulars pulled from the well. The rig, water tanks, two pumps, crane mats, and tubular are still remaining on location.

The rain is forecasted to continue the remainder of the day so the decision was made to postpone the cementing of the well till Saturday, June 24th.
0518655999 2017-07-19 Final Restoration No Violations On location to check the status of the restoration of the Brenneman Orphan Well Emergency Project. A total of 97 Tons of #57 stone was brought to dress the access provided by Hempker Farms. Most of that stone has been spread on the access with a pile left to fill in the low areas. The contractor still needs to bring in approved resoil material for the Brenneman farm field, and the Portable toilet and hydraulic pump need removed from the staging area.
0696375270 2017-06-24 Plug / Plug Back No Violations On location for the plugging of the Brenneman Orphan Well by Hydrocarbon Well Service. The Contractor ran three joints of 7 casing which were welded together after removing the threads. The casing stopped at 91 feet and a collar was welded to the top. A cementing head was then put on the casing and the well was circulated with fresh water. A slurry of 14.8 pounds per gallon cement was pumped down the 7 inch casing and back up the annular space between the 7 inch and the existing 8 inch conductor casing. The plug took 29 sacks total and the pumping was stopped after five minutes of cement returns to surface. The well was shut in and the annular cement fell back to approximately 14 feet. The well was left shut in overnight.
0861498645 2017-05-02 Orphan No Violations On location for the start of the Brenneman Orphan Well Emergency Plugging Project by Hydrocarbon Well Service. On Thursday, April 27th the field was planted with corn by Brenneman Bros. Inc. then 1.5 of rain fell over the weekend of April 29th and 30th. When the crew from Hydrocarbon Well Service arrived on location to mobilize equipment to the well late in the day on Monday, March 1st. On March 2nd, the mats designated for the east/west lane to the well over the field tile and around the well were set. The decision was made by the Contractor and the Division, through Field Order #1 and Change Order #1, to have mats delivered to set on the north south lane before bringing the rig and other equipment back to the well.
1474594234 2017-04-10 Orphan No Violations On April 10th, 2017, DOGRM Inspectors John Williams and Ben Harpster were on location. At 11:30 am,

after following the contaminated drainage tile, the well was discovered leaking into a four (4) inch diameter clay tile fifteen (15) feet to the east and in a four (4) inch diameter plastic tile less than a one foot west of the well. These tiles are draining north into six (6) inch dimeter tiles draining west to the main ten (10) inch trunk drain leading off the property. The well was located approximately three feet below grade with a rock placed on top of it. The well is equipped with an eight (8) inch diameter casing in poor condition. There was no evidence of any other casing, rods, lines, tanks, or any other production equipment. The well was then uncovered and immediately began to spill oil over the top of the casing. Eighteen (18) inches of casing was exposed, which began to fill with oil. There were two, three (3) inch by one (1) inch pieces of wood approximately eighteen (18) inches long that were removed from the top of the wellbore. A bale of peat moss was used to soak up the pooling oil before an eight (8) inch layer of Ben-Seal (granular bentonite) was placed over the peat moss. A twelve (12) inch diameter, seven (7) foot long plastic riser was placed over the well casing and additional Ben-Seal was used to fill the annulus between the well and the riser.

Installed Bentonite clay at base of tile approximately ten inches below wellhead. Liquid in tile rose to about ground level and did not rise any further. Will monitor well in near future.

1498487564 2017-06-21 Orphan No Violations On location for the cleanout of the Brenneman Orphan Well by Hydrocarbon Well Service. The Contractor continued fishing with a 7.375 inch outside diameter, 6.2 inch to 2 inch, box tap tool with no success. The decision was made to stop all fishing and milling and plug the well at the current depth of 95 per Field Order #11. The Contractor began breakdown of all equipment not necessary for the cementing of the well.
1498746059 2017-05-05 Orphan No Violations On location for the cleanout of the Brenneman Orphan Well by Hydrocarbon Well Service. After 1.5 inches of rain overnight, the well was excavated and an 8 inch to 8.625 inch dresser sleeve with an 8.625 inch collar to bring the well to ground level. The area around the well was backfilled but a small lined cellar was left encircling the well. An 8.625 inch to 2 inch fitting was placed on the well and the well was shut in. A load of fresh water for drilling and well control was delivered to location. All tanks, pumps, and the rig were set on 20 mil liners and Site Safety was completed on Saturday May 6th.
1525442820 2018-05-02 Final Restoration No Violations At the time of the inspection the Allen #1 Orphan Well Project is completed. The well was plugged on 6-25-2017 and the area has been reclaimed as an agricultural field. The field is currently planted with this years crop.
1647168377 2017-04-09 Orphan No Violations On April 7th, 2017, a sheen of oil was found approximately a half of mile north of the Brenneman Bros Inc.

Property just south of State Route 309. The Ohio Environmental Protection Agency (OEPA) was dispatched to handle the initial cleanup and determine the source of discharge. The Division of Oil & Gas Resources Management (DOGRM) Emergency Response Unit was contacted and DOGRM Representative Ben Harpster was on location on April 9th, 2017. The source of the contamination was determined to be from a ten (10) inch diameter plastic tile that is the main trunk drain for the Brenneman Bros Inc. property. There is historical evidence of wells being on the Brenneman Property but no wells are visible above grade and further investigation is needed to determine the source of the oil.

1724800592 2017-05-08 Orphan No Violations On location for the cleanout of the Brenneman Orphan Well by Hydrocarbon Well Service. On Sunday May 7th the cleanout of the well began by drilling out 7 feet of wood from 10 feet to 17 feet with a roller cone bit. At 17 feet a harder obstruction was encountered. The contractor switched to a 5.5 inch taper mill bit and drilled for an hour. When the pipe and bit were tripped out a 15 foot piece of 3.9 inch outside diameter casing was stuck to the bit. The pipe appeared to spin out of the pipe stil in the hole. On Monday May 8th the contractor continued to attempt to get inside the 3.9 inch casing with no success. At this time fishing tools were requested to remove the 3.9 inch casing.
1873748904 2017-06-20 Orphan No Violations On location for the cleanout of the Brenneman Orphan Well by Hydrocarbon Well Service. The Contractor continued fishing with a 7.375 inch outside diameter, 6.2 inch to 2 inch, box tap tool. A three foot long piece of rolled and twisted pipe that took almost two hours to remove from the tool. A second piece measuring four inches by two inches was removed during a later run. No other pipe was removed from the well.
1880961717 2017-05-04 Orphan No Violations On location for the Brenneman Orphan Well Emergency Plugging Project by Hydrocarbon Well Service. The rig mats requested through Field Order #1 and Change Order #1 were placed yesterday and the remaining equipment is being set up on the well today. Jason Simmerman of the Division of Oil & Gas Resources Management Orphan Well Program was on location to review the mats before heading to the Village of Ottawa Hills to review another possible Orphan Well location.
