1826809930 2015-09-08 Administrative Inspection No Violations I met with the landowner, John Darlak, who is operating the well for domestic purposes. I advised him on well registration and owner registration requirements. He advised that he is operating the well and that he is doing the maintenance on the well. He also advised that the well was recorded on his deed at the court house and that he has adequate insurance. Tom Benko of ODNR previously told Mr. Darlak to register as a well owner and register the well. Mr. Darlak was advised to contact the ODNR Columbus office and speak with the domestic owner registration agent. Mr. Darlak may be reached at 419-945-2175.
2055999207 2015-04-08 Production Wells No Violations The well is supplying house gas to the landowner. The well is not plugged. The landowner lives in the farm house on CR 500 south of the well. The well has been serviced recently and there is a newer swage and ball valves on the production casing. There are old pipeline regulators at the location which are not in use.
