Appalachian Well Service was contacted to shoot tubing off. They were only able to get tools into the tubing to a depth of 497ft. The job was put on hold until they can obtain a hot oiler or steam truck to clean the tubing or obtain 1.5 inch tubing to run in the well in order to get tools inside the production tubing.

0480447365 2014-03-18 Production Wells No Violations The well was inspected to determine compliance with Chiefs Order 2014-35. The well is not capable of production. The pump jack is overgrown with trees and brush and there is no motor on the jack. The well head is leaking gas and fluid which is contained at the well site. Clear Creek Oils owner resides in South America. The farm on which the well is located is occupied by the owners daughter. Their legal affairs are managed by Critchfield and Critchfield attorney Patrick Noser (419-289-6888). Their administrative business is handled by Whitcomb and Hess accountants. The representatives of the company were notified via phone on 3/19/14 that the well will need to be plugged. They are working on the delinquent insurance and then they will work on plugging the well. Compliance Notice will be issued if there is no progress on plugging or repairing the well.
0656850116 2014-11-20 Plug / Plug Back No Violations 3 cement plugs were placed in the well using tubing. A plug was placed across the failed 5.5 inch production casing, at the bottom of surface casing and at surface. The surface plug fell away following pumping. The well was topped up once and cement fell away and subsequent attempts did not fill up the well and the contractor ran out of cement. Cement will be tagged in the morning. No cement was visible on the backside of the surface casing when the plugging contractor excavated around itThe well will be topped up with cement tomorrow. See the plug report for plug details.

1416516062 2014-11-18 Plug / Plug Back No Violations Nicks Well Plugging and Sutton Pump Service completed steam cleaning of the production tubing. Wireline with a casing cutting tool was ran into the production tubing to cut the tubing. The tubing was cut at 3700 ft. There may be a packer on the bottom of the tubing, but the rig crew was unable to get it to release. The production casing was then perforated at the Trenton formation and a bottom plug was placed through the tubing. See the plug report for further detail.
1416845080 2014-11-21 Plug / Plug Back No Violations Cement was tagged with a weighted line at 54 ft. Fluid was encountered at 24 ft. Nicks Well Plugging topped up the casing starting with 1 bag of hulls to attempt to bridge the thief zone and then dry cement. The casing was filled up to surface with dry cement and topped up twice. Water was added after cement stopped settling and cement dropped approximately 5ft and held at that level. See the plug report for further details.

1719781659 2015-02-10 Final Restoration No Violations A landowner waiver was received from the property owner indicating that he approves leaving the flow line in the ground. The pump jack was removed from the well and stored at the tank battery for the remaining 2 wells on the property and has been sold. The buyer is waiting on weather to improve before they pick up the pump jack. The pit was closed, the location was graded and the casing was cut off concurrent with plugging. Nicks Well plugging placed a steel plate with the permit number on it when the surface plug was finished. Final restoration is approved.
1770924025 2014-11-19 Plug / Plug Back No Violations The production tubing was recovered from the well. Nicks Well Plugging attempted to use a wireline company to cut the 5.5 inch production casing at the base of the lime to place a rip plug in the well to isolate the Big Lime formation. The 5.5 inch casing was found to be parted above the lime. At that point, it was decided to recover the 5.5 inch above the failed section and run tubing back into the well to complete plugging. Tubing was ran to a depth of 1023ft, casing top was at 926ft.
