1461773268 2016-04-26 Production Wells No Violations Employees at the Pentair factory discovered this well leaking gas and detected gas odor. They contacted Columbia Gas of Ohio to investigate the leak. Columbia determined that the leak was from the well and was not from their facilities. They contacted the Division inspector on 4/26/16. Mike from Columbia of Ohio (419-460-3449) took the inspector to the well on the same date as the complaint. Pentair/Myers Pump is the landowner. Dave Christine is the maintenance manager at the factory. He can be reached at 419-281-9962. Doug Clark, maintenance supervisor was present during inspection. The well is equipped with 8.625 inch casing. There is a fabricated head on the 8.625 inch casing. The head consists of a piece of 8.625 inch casing with a plate welded to it and a 3.5 inch casing welded to the plate. There is a swage and valve on the 3.5 inch casing. There is no flowline or equipment at the well. The well is leaking gas from the collar threads where the wellhead connects to the surface casing. The leak is audible. No gas odor was observed around the well during inspection. There was no way to take pressure readings on the well. There are plugging records in RBDMS images from 1975. The original owner is identified as Tom Younkin. There is an obituary for a Tom Younkin from 2000.
