0259217634 2014-06-19 Production Wells No Violations The landowner and operator are working on transferring the well registration to the landowner. They have been in contact with Erica Freeman at the Division permitting office. The landowner has an owner registration. Killbuck Creek has failed to sign transfer paperwork to the landowner previously. There may be an issue transferring the well due to an ID violation issued by Harold Moravy in 2009. If ID is posted at the well, the violation will be terminated and the transfer can proceed. There was a court case involving this well and the settlement appears to place ownership of the well with the landowner. Messages were left for Erica, the operator and the landowner to discuss this well.

A gauge at the wellhead indicates approximately 500psi. There are no compliance issues at the well other than ID, which will need to be posted following registration to the landowner.
1371831946 2013-06-20 Production Wells No Violations No action has been taken on the NOV issued by Harold Moravy. No ID posted. The well is supplying house gas to the residences on the property. The homeowner states he cancelled the lease through the court house in Ashland. He was told again that he needs to register as an owner of the well and register the well in his name. Jim Shoots of Killbuck Creek previously stated he was willing to transfer the well to the surface owner. Nothing has been done to date to initiate a transfer. Killbuck Creek is not maintaining the well.

The well is supplying house gas to the Lowe residence. The Lowes believe the settlement of a court case granted them ownership of the well; ownership has not been transferred. The well is non compliant, plugging required under a compliance notice issued on 7/16/14 was required to be completed by 1/16/15. A Chiefs Order will be prepared.

They had the lease cancelled in the Ashland County Courthouse and the disposition of the case put the well ownership to the landowner. The company has been unwilling to sign the paperwork to change the ownership of the well with ODNR. The surface owner has an owner registration but the well is not in their name. Jim Shoots of Killbuck Creek conceded that the lease is no longer valid and that he may be willing to sign the well over to the surface owner if they cant reach any other agreement.
1759687939 2014-09-03 Production Wells No Violations There is no ID posted at the well to date. The well has not been transferred to the property owner. Jim Shoots at Killbuck Creek Oil stated via phone on 9/3/14 he has been unable to install pipeline to the well and has not done any work yet. He stated he would not post ID until he installs pipeline. The well is supplying house gas to the landowner who has previously indicated that they want to take it over following legal proceedings to forfeit the lease for the well.
1869746282 2013-03-13 Production Wells No Violations Follow up to NOV issued by Harold Moravy in Feb. 2009. I spoke with the surface owner, Mr. Lowe, who advised that he has taken over the well. Jim Shoots of Killbuck Creek Oil advised that the landowner just wont let them on the property and has taken no action to formally own the well. I will be providing Mr. Lowe with a domestic owner packet. They are taking house gas off the well. Mr. Lowe is maintaining the well head. The tanks and production unit are off line. An ID needs to be posted at the well location, however I told Shoots that we can try to work out ownership transfer to the landowner prior to posting well ID.
1928909280 2014-02-18 Production Wells No Violations The surface owner wishes to take ownership of the well and has court paperwork indicating that the operators lease is invalid. The well has not been transferred yet.
1973900943 2015-11-05 Production Wells No Violations There is a swage on the production casing with a gate valve on it. The valve is open to the flow line. A guage on the wellhead is showing slightly over 500 psi. The production unit and tank are shut in. The well is supplying domestic gas to the landowner. The well remains unidentified. No additional production reports have been filed since 2012. The landowner forefeited the lease in the Ashland Courthouse. I met with the landowner and Bill Kahelin, their attorney during inspection. They are attempting to register as the owner of the well but Killbuck Creek will not sign a form 7. The operator has previously indicated willingness to transfer the well but they have not signed the transfer to the landowner. The landowner has indicated that they are performing all service on the well and that Killbuck Creek has not been around. I advised them to work with Erica Freeman, ODNR to complete transfer of the well. They believed that an owner number previously issued to them and the last round of attempts had satisfied transfer requirements. The landowner was advised that forefeiting the lease was not sufficient to transfer the well and the court would have to specifically grant them title to the well to register without Killbuck Creek signing the form.
2109122568 2014-07-15 Production Wells Violations Noted The operator has not posted ID at the location to correct the compliance notice from 2009. The landowner is interested in ownership of the well and is a registered owner but they have not changed the registration of the well yet. The well is supplying house gas to the landowner. The transfer of registration has not been completed and the operator is not maintaining the well. The well needs to be plugged and the location restored by January 16, 2015.
