0083743278 2019-04-18 Production Wells No Violations Inspected production well on 04/18/2019 to follow-up on a previously-issued compliance notice ordering the well to be plugged. The tank battery consists of 1-100 bbl steel tank and 1-horizontal separator with a drip and meter box, located within an earthen dike; 1-meter with a drip and regulator is located nearby. Well identification is clearly displayed at the tank battery. The well is equipped with a threaded wellhead with a swedge to the production casing, and a plug. A flowline is present, but not hooked-up. Doug Hansen (Bass Energy) was notified of the outstanding compliance notice and advised to either apply for temporary inactive status or determine a plan to plug the well promptly. No other problems were noted at the time of the inspection.
0336149833 2015-06-01 Production Wells No Violations The well is shut in. There is 4.5 inch production casing in the well. The flow line is not connected at the wellhead. No gas was detected around the well. The area is marshy floodplain and the well and tank battery are overgrown with vegetation. There is a 100 Bbl tank and a separator in the tank battery. The tank is rusty but it is empty. The lease road to the well and tank battery is not maintained and is impassible in a vehicle. The operator has reported 0 production for the last 2 years. The operator advises that the controlling interest investors have not decided how to recomplete the well following plugging it back to the Ohio Shale. They are going to put together a timetable to complete or plug the well.
0639894134 2015-10-15 Production Wells Violations Noted The operator previously indicated they would furnish a timetable for completing or plugging the well. To date none has been received by the Division. The tank battery and well remain overgrown with vegetation. The lease road is rough to non existant. The well is disconnected from the flow line and is idle. Ralph Gregory said they would work on submitting a schedule to the division. He has to speak with Bill Lavin on Monday. The current state of the well is 4.5 inch production casing cemented to surface. The casing is not perforated. The well owner/operator shall submit a written plugging schedule to Tom Zacharyasz at 116 East High St, Mt. Vernon, OH 43050 within 30 days, per OAC 1501:9-11-05(B), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied.

Bill at Bass Energy was contacted regarding the well. He stated they would still like to perforate the lime and return the well to production. 1131188115 2012-07-24 Production Wells No Violations At the time of my inspection I was on site to observe the well operator investigate a gas leak in the Surface Casing. A hole was discovered in the Surface Casing two feet below ground level.
1344338394 2012-07-31 Plug / Plug Back No Violations As a result of Hydrogen Sulfide breaching the Surface casing the operator intended to plug back this well to the Bois Blanc Formation. During construction of the well, the well was perforated from 1950 ft. to 1958 ft. a bridge plug was set at 1945 ft. The well was then perforated from 1930 ft. to 1938 and a bridge plug was set at 1921 ft. The well was then perforated from 1904 ft. to 1908 ft. and this zone was put into production. Cement Bottom plug was set through tubing from 1908 to 1708, a Mechanical bridge plug was set at 1433 ft by Appalachian Geophysical Service. This was designed by Bob Worstall. The 4.5 inch casing was shot off at 1250 ft. A cement rip plug was set from 1433 to 1150 through tubing.

The temporary inactive was not renewed and the well has not been placed into production. The well was previously plugged back to the lime formation but it was not recompleted. The well is not currently capable of production. Ralph Gregory at Bass Energy was contacted via phone to discuss plans for the well. The well owner/operator shall submit a written plugging schedule to Tom Zacharyasz at 1656 Eagle Way, Ashland Ohio within 30 days, per ORC 1509:13(B)(7), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied.

Collect gas readings with gas meter. CH4=3.8, LEL= 80%, CO=7 ppm, and H2S=108 ppm. Tank is heavily rusted below the overflow port at top of tank. Well is shut in.
