1190658871 2007-09-24 Preliminary Restoration Violations Noted Complaint of an oil spill by USEPA, James Augustyn, in Mohican River, Lake Township, Ashland County. OEPA, Wes Drake was notified. I arrived on scene with Dick Marshall ,operator of the oil well. We walked the channels leading to the Mohican River and found no traces of oil from the well in question. Wes Drake arrived and followed up. I advised Dick Marshall to close drilling pits as soon as possible, due to rains coming this week. Pits were contained and capable of holding more fluid. Area inside dike has visible oil stain and advised him to clean any visible oil contamination around dike or location. River is 700 to west of tank battery and well location. Dick has drilled several Berea wells in this area. Dry, Sun, 80 degrees.
1283263608 2010-08-31 Production Wells Violations Noted Oil spill inside dike in water and 4 x 5 area around well head. Well is in production. Remove all oil, diapers, and contaminated soils inside dike and pump jack area. Haul to approved site and dispose of properly. Gave verbal NOV on 8-21 2010. No response. Drilling pit for well 12 has a hole in liner. Pump pit and line with new liner before drilling commences. Sun, dry, 75 degrees.
