0204823825 2015-08-31 Plug / Plug Back No Violations Nicks Well Plugging (NWP) pulled the bit from the well after hitting a solid obstruction at 710 feet. The bit had metal fragments in it and some wood fragments. Drilling was making no downward progress. NWP considered running a spear into the well and decided against it for concern that they would become stuck in the 7 inch casing. Shale cuttings were coming back in fluid returns indicating possible wall caving or washout since the bit is on hard object. An impression block was ran in the hole. The block showed nothing but some dimples on the face of it and it was mud covered. The bit was washed off and found to be galled and missing carbide buttons from the sides of it. More metal pieces were found in the bit. After consultion with Will Ziegler, Bob Worstal and Dave Claus, ODNR, a decision was made to plug the well solid with cement from the current depth. NWP does not believe they can drill past it, push it down hole or fish it from the well. The obstruction is believed to be lost tools as the metal fragments are heavier steel than the cable previously encountered.

0934176974 2015-08-19 Plug / Plug Back No Violations Nicks Well Plugging (NWP) called on Tuesday 8/18/15 and stated that they were having integrity problems with the borehole and circulation was returning outside the old 10 inch drive pipe. They stated they were going to have to set surface casing and requested a depth to set pipe. Steve Opritza of ODNR Permitting office in Columbus advised 200 feet would be sufficient for plugging operations. NWP stated they would get 7 inch casing and be ready to plug on Wednesday, 8/19/15. The inspector was in a meeting on Tuesday and was not present at the plugging job. On Wednesday, NWP attempted to install the 7 inch casing in the well by pumping water through the casing as it was lowered into the borehole. At approximately 100 feet down, the casing encountered an obstruction and they were not able to rotate or wash past it. Wood was coming up in the circulation returns. The inspector had to leave early for a doctor appointment and Allie Sherwood witnessed tripping the casing out of the hole and running the 3 inch bit back in. She advised at the end of the day that no further progress had been made. NWP is using a cement pump truck to circulate the well with fresh water. They switched to fresh water from saltwater when they determined they need to set surface casing. Saltwater was being used initially to prevent problems drilling through the Bedford shale. They dont have gel on site or lost circulation material available.
0968973828 2015-09-01 Plug / Plug Back No Violations Tubing was ran into the well and encountered solid bottom at 705 feet. Tubing was pulled off bottom and circulation established. 115 sacks of cement was pumped for a 400 foot plug. Calcium chloride was added to the cement. Tubing was pulled from the well.
1081087717 2015-09-10 Plug / Plug Back No Violations The location is finish graded. The disturbed area is bare and will be used as hay field. Sunoco inspector Jon Bell was on site with the reclamation contractor and landowner discussing final restoration of the access and well area. They are seeking additional money from GES concerning final restoration of the location. They were provided contact information for Bryan Burtscher, GES inspector. They alleged gas odor in the vicinity of the well. I did not observe gas odor nor did the gas detector register any levels of methane or LEL. The well was not observed to be leaking following plugging or prior to cutting off the casing. The detector was calibrated 9/9/15. There was a strong north wind at the time of inspection, approximately 3:40 pm. The Sunoco inspector requested information concerning ODNR revegetation standards.
1083082952 2014-04-23 Orphan No Violations Inspect well for plugging operation. Well is located within construction zone for a replacement petroleum pipeline. Ten inch casing is only visible casing at surface. Thick black oil is present at a depth of 18 feet below grade. Records have been located to indicate the well was plugged in 1932. There was 1830 feet of 6 1/8 inch casing originally installed. The plug report indicates that 1764 feet of 6 1/8 inch casing was removed.
1190437571 2015-08-21 Plug / Plug Back No Violations Nicks Well Plugging (NWP) arrived on location with a mini excavator. A well with 6.5 inch casing and a plastic cap on the casing was discovered on the north side of the 8 inch pipeline and about 5 feet to the north of the oil and gas well. The top of the casing was slightly more than 3 feet below grade. NWP ran tubing into the well and tagged the bottom at 70 feet. This depth corresponds to water well depth in the area, and without detailed records, is presumed to be the total depth of the water well. The oil and gas well was shut in using a swage on the field fabricated well head. The wellhead consisted of 9 inch casing welded into the old 10 inch drive pipe and 8 inch welded into the 9 inch with a collar on top. The water well was plugged with cement displaced through tubing. 13 sacks was calculated fill up volume. NWP reported pumping 60 sacks of cement before cement returned to surface. During pumping, fluid was leaking from the 10 inch casing in the oil well borehole under pressure. Cement was left to set up before downhole operations resumed. A pit was excavated on the north side of the old pipeline and lined.
1220217940 2015-08-20 Plug / Plug Back No Violations Nicks Well Plugging was on site in the morning rigging up a 7.875 inch bit to clean the hole out to run 7 inch surface casing. The inspector had a fracture stimulation job scheduled as well and left to witness it. NWP was advised to contact the inspector before running casing or if any issues were encountered. A call was received at approximately 11 AM from the service rig stating that Sunocos Environmental inspector shut them down due to circulation returning through the ground surface about 3 feet from the well bore. I contacted Tom Benko, ODNR who met me at the location. We discussed setting the 7 inch casing shallow at 110 feet to seal the well bore up with Steve Opritza who agreed to allow that because the deepest water wells in the area were at 70 feet. Will Ziegler, ODNR Northwest supervisor was contacted and denied permission to set casing at 110 feet. The rig crew was instructed to clean out the borehole and run 7 inch to 200 feet. They resumed drilling using gel and cottonseed hull LCM in their drilling fluid to attempt to condition and seal the borehole. Circulation again returned through the ground surface about 3 feet from the borehole and begain increasing in flow rate. Drilling was stopped again. Bob Worstall, Deputy Chief, ODNR and Steve Opritza, ODNR were consulted with as well as Todd Miller, ODNR Inspector and Will Ziegler. At this point a water well was the suspected conduit for fluid returning to surface. Sunoco Pipeline was consulted to allow excavation to locate the suspected water well. After consulting with their supervisors, they agreed to allow excavation to the north side of the well using a mini excavator with the stipulation that the old 8 inch pipeline not be beat up. Downhole operations in the well were suspended at this point. Nicks Well Plugging suggested bullheading cement down the 10 inch casing to attempt to seal fluid loss zones and then drilling it back out, but permission to do so was denied by Will Ziegler. N
1316298421 2015-09-02 Plug / Plug Back No Violations Cement was tagged with tubing at 460 feet down. Cement either was absorbed into the Berea formation, drifted down hole or the hole was larger than the 7.875 inch diameter used to calculate cement volume. Planned cement top was 305 feet. One joint of tubing was pulled and circulation was established. Cement was pumped until it returned to surface. At the end of the day the well was checked for cement loss and gas leakage. Cement was holding at surface. No leakage was observed and no gas was detected on the gas detector around the well or at the top of the 7 inch casing.
1357502899 2015-08-25 Plug / Plug Back No Violations Nicks Well Plugging (NWP) ran an impression block down hole to try to determine the shape of the obstruction. The results were inconclusive. There were small holes in the block but no definitive impression. The 3.5 inch bit was reinstalled and ran down to 110 feet. Wire fragments were found in the bit. The spear was installed and ran down the hole to 110 feet. Nothing was pulled up on the spear. A 6.5 inch bit was installed and ran down to 110 feet. Metal shavings and cable fragments were found in fluid returns. Drilling began to move down hole to 160 feet. Progress is slow and the rig operator advises that the obstruction is taking weight off the block on the rig. Wood chips were found in fluid returns. The rig operator believes they are pushing the obstruction ahead of the bit. The bit was pulled from the well and the spear installed. A wad of cable with lumber tangled in it was fished from the well bore.
1389027008 2013-12-31 Plug / Plug Back No Violations Inspected well location. Landowner located and identified 10 inch casing with oil staining at surface. Ten inch casing is present at surface with oil staining. Appears well has been plugged and all casings have been removed. Clay plug has dropped about 15 to 20 feet. Slight odor of gas. Inspected the Ohio Fuel Gas Maps; the map symbols indicate the well was plugged. The status of this well needs to be changed to plugged. The well needs to be re-plugged. This well is located within one foot of an existing 12 inch petroleum pipeline that is reported to be removed within 6 months and on the pipeline easement.

The top of the well is deeper than the bottom of the old 8 inch pipeline. Moore Well Service was on site with a mini excavator. Cement was about 2 feet below the cut off casing. Bagged cement was used to top up the well before the plate was welded on the casing top.
1513997081 2015-08-27 Plug / Plug Back No Violations Nicks Well Plugging (NWP) ran the 7 inch surface casing in the well to a depth of 210 feet. The well bore was circulated with fresh water prior to cementing. The casing was cemented with 50 sacks of cement. Cement did not return to surface; circulation was maintained. Fluid level on the backside of the 7 inch dropped following pumping cement. 4 hours after cement was pumped, a weighted line was dropped down the backside of the 7 inch casing by Tom Benko, ODNR. Cement top was encountered at 40 feet down. Cement will be topped up by NWP in the morning.
1663018355 2014-02-04 Orphan No Violations An inspection of this well was conducted in response to a referral received from Oil & Gas Inspector, Tom Benko. This inspection revealed an open wellbore that was leaking natural gas to the surface. The wellbore measured 4 diameter at the surface, but was larger in diameter below ground level. Note: Winter Weather conditions at time of inspection. A visual observation of this wellbore beneath the surface showed that steel casing appears to exist. A metal detector was also utilized and verified the potential presence of steel casing. An available Well Record indicated that the wellbore had contained 10 drive pipe. The total depth for this wellbore was measured 18, but recommend the use of a heavier weight for an accurate depth. During the retrieval, it was noted that the weight contained crude oil. The leakage of natural gas from the wellbore could be heard at the surface and was also monitored with a gas meter. The meter detected a maximum level of CH4 and LEL from this well. The was situated on a property designated as agricultural field and located next to a buried Sunoco Petroleum Pipeline. At the conclusion of this inspection, Tom Davis with Lineal Industries was present and confirmed that his employer would be installing a new pipeline on the property for Sunoco within the next two months. A GPS reading was obtained for a potential entry point.
1667481958 2015-08-28 Plug / Plug Back No Violations Cement on the backside of the 7 inch casing was topped up with 5 gallon buckets. Fluid level is at 4 feet down. Nicks Well Plugging (NWP) began drilling out below surface casing with a 6.25 inch bit and power swivel. Cement was encountered in the 7 inch casing at 205 feet. Cement was drilled out and the bit ran to 570 feet. At this depth the hole was tight on the bit and drilling slowed down.
1694223942 2015-08-24 Plug / Plug Back No Violations Nicks Well Plugging (NWP) ran the 7.875 inch bit into the oil well and encountered solid cement at 100 feet. Cement was drilled out and the previously encountered obstruction was struck at 110 feet. Drilling was not making progress. NWP pulled the bit out of the well after discovering metal fragments and pieces of wire in the fluid returns. A spear tool was ran into the well and a large wad of 7/8-inch left hand twist cable was recovered from the well. NWP is planning to run an impression block, spear and possibly a mill in the morning. Operations shut down for the day.

2078720709 2015-08-17 Plug / Plug Back No Violations Nicks Well Plugging (NWP) moved their service rig in to begin plugging the well in the morning. Sunoco Pipeline inspectors would not allow them to rig up until the rig location was approved by their bosses. The rig was set up and in the afternoon was drilling with a 3.5 inch bit between 640 and 670 feet. The well needs to be reopened to plug properly. Sunoco representatives and Groundwater Environmental Service inspector on location. NWP is using a power swivel for drilling on the service rig and they are using a cement pump truck for circulation. They have a fluid tank on site from S&H Oilfield Service and a tank for returns to circulate into. They are using saltwater for circulation. They are hauling water with their water truck.
2140006792 2015-08-26 Plug / Plug Back No Violations Nicks Well Plugging (NWP) ran in the hole with a fishing tool made from 7 inch casing with barbs on the inside of the tool. An obstruction was encountered at 160 feet but nothing was retrieved by the tool. The spear was ran back down the hole to 619 feet. Upon pulling the spear from the wellbore, it lodged in the wellhead. The rig operator pulled 35,000 pounds on the block at which point the spear broke and came out of the hole. A large wad of cable was stuck at the 8 inch casing in the wellhead. Cable was unraveled from the bundle and the wellhead was then cut off to remove the cable. No more debris was pulled from the borehole with the spear or the 7 inch junk basket. The 7 inch tool was ran to 580 feet and the spear made it to 630 feet. The spear encountered a soft obstruction at 630 feet and did not recover anything from the wellbore. The spear was covered in gray mud with shale fragments in it. No oil was on the spear.
