0200560060 2015-02-25 Production Wells No Violations The well is equipped with a pump jack and is in production. ID posted. There are 2-200 Bbl tanks and a vault in the tank battery. The vault is due to be replaced with an above ground tank inside the concrete containment structure. The well was pumping during inspection.
1238084890 2009-03-26 Production Wells Violations Noted Landowner was plowing thru waterway in field and broke an underground production line leading from well to tank battery. Crude entered field tile and drained in stream leading to the Muddy Fork River. Len Rex called Division Inspector. I advised to call OEPA due to oil in stream. Owner of company Randy Metheny did not call EPA until 3-26-2009. Spill occurred 3-25-2009. Siphon dam was constructed in stream after I responded to site on 3-26-2009. Oil sheen remains on water from entry point to siphon dam 300 down stream. I walked downstream 1000 and no sheen was visable below dam. Approximately 3 barrels of oil was reported spilled. Photographed stream for EPA and e-mailed. Wes Drake of OEPA responded later. Rain, 40 degrees.
1238501137 2009-03-31 Production Wells No Violations Crude oil is still collecting in siphon dam. Debris has been removed out of stream to allow oil to release. Cleanup to continue on creek bank. Sun, 30 degrees.
1238769822 2009-04-03 Production Wells No Violations All contaminated soil has been removed and graded. Stream is cleaned and siphon dam partially removed. OEPA to instructing company on dam removal.Leak was repaired. NOV in compliance. Rain, 50 degrees.
