0047046001 2015-05-04 Production Wells No Violations Harley drilling has posted identification on the well. The well is equipped with a pump jack, rods and tubing and is in production. The well that was previously inspected with this API has been identified by the operator as permit 2-2108.
0539730076 2017-02-07 Production Wells No Violations Heavy rain happened over night into the morning. The rain floated some oil off the soil next to the well. It is collected in puddles at the location. The pumper is responding with some absorbent pads. Ground conditions are very soft and muddy.
0697832741 2017-02-06 Production Wells Violations Noted A call was received from the landowner, Jack Morris, by the Northwest Region office stating oil was not satisfactorily cleaned up around the well. On inspection, oil was observed leaking from the stuffing box on the well head and oil was observed floating in two small puddles to the east side of the well. The pumper for Harley Drilling was contacted and cleaned up the puddles and wellhead. Oil staining was observed on the soil. Remove oil soaked soil from the well location by 2/20/17.
1140498130 2015-02-17 Production Wells Violations Noted The well is equipped with rods and tubing. There is a pump jack matching the colors of those used by Harley Drilling at the well. The bridle is unhooked from the pump jack and the drive belt is off the motor. The well is unidentified and is not in production. 3-100 Bbl tanks are located in the tank battery. Post identification at the well and register the well by May 4, 2015.
1145130538 2016-04-08 Production Wells No Violations There is a large area around the well that has been disturbed. The operator previously had a service rig on the lease. A D5 dozer is still on the farm. The area around the well has been scraped by the dozer. Todd Miller of ODNR received a complaint alleging fluid being swabbed from the well and dumped on the ground by Harley Drilling. The rig is no longer on location. There is crude oil or paraffin on the well head. No evidence of an oil spill or dumping was observed. The soil is black in color and mucky. No sheen was observed on the water and no oil soaked soil was observed around the well. The flowline was exposed by the grading activities around the well and appears intact. The well is equipped with a pump jack, rods and tubing and is in production. ID is posted.
1296197209 2017-02-14 Production Wells No Violations The operator removed oil contaminated soil around the well and applied pelletized lime to the area. The leak was previously repaired. The well is in compliance.
1486491615 2016-12-05 Production Wells No Violations A voice message was received from the landowner at 7pm Friday, 12/2/16 concerning oil leaked from the well. The pumper, Jim Jenkins, stated in a phone call that the leak was from the stuffing box and the well was shut in. He also stated he had absorbent pads on the spilled oil. The well was not actively leaking at the time of the call. Northwest Region Supervisor Will Ziegler was contacted and it was decided to complete cleanup Monday, 12/5/16. Harley Drilling cleaned up spilled oil and removed contaminated soil. The oil volume amounted to less than 5 gallons. The pumper was on site during inspection. The stuffing box on the wellhead was the source of the leak. The stuffing box was repaired and the well was turned on. No oil was observed leaking during well operation.
