0482802859 | 2017-09-26 | Production Wells | Violations Noted | The Novak # 1 was accessed on a two (2) tire path through an hay field. The well was operated as a 2.375 plunger lift well. The wellhead valves were shut in on the tubing and casing. I did not identify any leaks at the wellhead. The Operator/ Owner last reported production volumes for the year of 2012 at 3 thousand cubic feet of natural gas. The tank battery consisted of one (1) 210 barrel stock tank, one (1) 100 barrel brine tank and one horizontal separator. It appeared that chain link fencing previously surrounded the tank battery but the fencing fabric was removed leaving the posts. The I D signage was a pair of 4 X 6 plastic cards with the well information. My inspection revealed a severely corroded 210 barrel stock tank bottom leaking oil into the tank pad. It appeared that the gravel was removed under the tank to assess the extent of contamination. There were several pools of free and mobile oil, one on the southwest side of the tank and another at the heat tube hatch. OAC 1501:9-9-05 (A) (8 & 9) states that all surface equipment must be pressure rated to withstand operating pressures to which it is subjected and in part The dike or pit shall be maintained for the purpose for which it was constructed, and the reservoir within shall be kept resonable free of water and oil. To bring this well into compliance the Owner / Operator shall remove the leaking 210 barrel tank, excavate and properly dispose ( supply DOGRM manifests ) of the contaminated material underneath the tank within two (2) weeks. Further compliance shall be met by the Owner / Operator submitting, within two (2) weeks, to the Division of Oil and Gas Resource Management at 3575 Forest Lake Drive, Suite 150, Uniontown, Ohio 44685, a plugging schedule for the Novak #1 and plugging the well within 180 days. The Division of Oil and Gas Management will review the plugging schedule and notifify Petrox, Inc. if it is accepted or denied. Please contact Bob Roush at 330.690.8837. |

At the time of this inspection, the well was equipped with a plunger lift assembly. The wellhead valves were shut in on the tubing and casing. There was legible identification located at the well. The tank battery production equipment, including the fence stakes, have been removed from location. The contaminated soils have been removed and at the time of this inspection, crops had been planted and harvested where the tank battery previously was installed. Petrox still needs to:
