Ms. Smith indicated that the 210 BBL tank on site had a previous leak and needed replaced. At the time of this inspection, the diked tank battery consisted of one 210 BBL tank, one 100 BBL tank and a mechanical separator. The 2 inch flow line on the inlet side of the separator was closed. The 210 BBL tank had been repaired where a previous leak had occurred and emptied. There was a new 100 BBL tank outside of the tank battery. Ms. Smith stated that the company had dropped off the new 100 BBL tank the week of July 30th to replace the 210 BBL tank. Evidence of a previous leak within the tank battery had been remedied. The tank battery was diked and had legible signage. The well was equipped with a pumping unit and the 2 inch valves on the casing and tubing were open. There was a slight audible gas leak coming from the tubing packing. I contacted Dennis Oil & Gas, LLC and spoke with owner Joe Dennis, regarding the gas leak. Mr. Dennis confirmed that the 100 BBL tank was onsite to replace the 210 BBL tank. The lease access was stable. |

We walked 250 feet east of the tank battery area along the lease road to a area that was scalded with oil soaked soil in the center of a area 20 feet by 6 feet. I then inspected the tank battery to find fresh oil lying on the ground at the base of the 100 barrel steel tank in a area 2 feet by 8 feet. The tank appears to be leaking. I also found a pvc pipe through the earthen dike wall on the west side which was open. There was a plug lying on the ground next to the pipe. REMEDIAL WORK REQUIRED TO RESOLVE THE VIOLATIONS: REMOVE ALL OIL SOAKED SOIL, REPAIR OR REPLACE LEAKING PRODUCTION TANK, REMOVE PIPE THROUGH DIKE WALL. |
