0048678403 2019-01-08 Production Wells No Violations I conducted a compliance notice follow-up inspection for compliance notice 1444155947, regarding the cleanup of the tank battery oil spill. During this inspection, efforts to mitigate the free oil inside the tank battery had not been conducted. The tank battery was diked but free oil was still present within the tank battery. The 100 BBL tank heater tube had not been repaired. The area approximately 9X27 feet outside of the diked tank battery had been previously impacted from the oil overflowing the dike on the west side. The impacted area outside of the dike was nonvegetated and stained. Previous cleanup efforts had been conducted by Charles Moisio. Identification has not been installed at the tank or well location.

0095140078 2019-03-28 Production Wells No Violations I conducted a compliance notice follow-- inch casing with a drip bottle and regulator. The tank battery consisted of one 100-barrel tank, one 50-barrel tank and a mechanical separator. An earthen dike surrounded the tank battery. The entire diked area was covered with oil and water due to a leaking heater tube on the 100-barrel tank. There was an area outside of dike approximately 10 foot by 10 foot that had oil on it due to the overflow of tank battery. There is a stream approximately 200 feet from the tank battery, in which the oil could possibly reach from the overflow of tank battery. Identification was posted at the tank battery. The lease road was in stable condition. I contacted Charles Moisio about the clean-up. He informed me the he does not own the well and landowner will not let him on the property.
1008683885 2016-01-15 Production Wells No Violations I was on site to conduct an inspection of this well. Oil around the wellhead and tank battery had still not been cleaned up at the time this inspection. Please see previous report dated 11/19/2015.

1289950447 2017-08-14 Production Wells No Violations I conducted a compliance notice follow-up inspection for this well. The well was equipped with a pumpjack and the valve off of the domestic line was open. The tank battery consisted of one 100 BBL production tank, one 35 BBL steel brine tank and a mechanical separator. Valves on the separator were open. The tank battery was full of water and oil and was contained within the dike. No changes have been made since the last inspection report dated 1/15/2016. Signage has not been posted on the well site. The lease access was stable. I contacted Charles Moisio of Plateau Energy about cleaning up the tank battery and removing all of the oil/water and impacted medium from inside the dike.

The tank battery was still full of water and oil. The dike was heavily eroded and there were areas of the dike which had been breached and still not repaired. Oil and water were present outside of the dike and the area adjacent to the dike was devoid of vegetation. Signage on the production tank was not legible and still needs replaced.

1444155947 2015-09-22 Production Wells Violations Noted I was on site to conduct an order follow-up inspection of this well. The well has a pump jack on it and is not in production. There is a domestic gas line. The stuffing box was coated with oil and paraffin. The tank battery consisted of one, 100 BBL production tank, a 1500 gallon steel brine tank, and a mechanical separator. The tank battery was full of water and oil. The dike was heavily eroded and there were areas of the dike which had been breached. Oil and water were present outside of the dike and the area adjacent to the dike was devoid of vegetation. I took a high range Quantab sample and the reading on the Quantab was 5. The signage on the production tank was not legible. The lease access was stable. The following needs completed by:

1) Clean-up all spilled oil/brine in tank battery and repair the dike (10-20-15) 2) Clean-up the parrafin around the wellhead and stuffing box (10-20-15) 3) A new identification sign needs posted (10-20-15)

1476062804 2018-08-22 Production Wells No Violations I conducted a compliance notice follow-up inspection for compliance notice 1444155947. The well was equipped with a pumping unit and the valves on the tubing and flow line were closed. There was an active domestic line plumbed off of the 4.5 inch casing with a drip bottle and regulator. The tank battery consisted of one 100 BBL tank, one 50 BBL tank and a mechanical separator. The tank battery was diked but free oil was still present within the tank battery. The entire diked area was covered with oil and water due to a leaking heater tube on the 100 BBL tank. An area approximately 9X27 feet outside of the diked tank battery had been previously impacted from the oil overflowing the dike on the west side. The impacted area outside of the dike was nonvegetated and stained. Previous cleanup efforts had been conducted by Charles Moisio, but the heater tube on the 100 BBL tank has never been repaired. Signage was illegible at the tank battery location and lease access stable. Efforts to contact Charles Moisio about removing the oil and water from the dike and repairing the leaking tank were unsuccessful.
1501128706 2017-08-15 Production Wells No Violations I conducted a compliance notice follow-up on this site for the previous inspection dated 8/14/2017. Met Charles Moisio of Plateau Energy on site to conduct the cleanup of the tank battery. Mr. Moisio was on site with a pump truck to begin the removal of the oil/water from the tank battery. Approximately 3 BBLs of fluid was removed from the tank battery. Mr. Moisio indicated that he would return to the site to remove the rest of the fluid and repair the leaking heater tube on the 100 BBL tank that had caused the release.

1631335236 2015-11-06 Production Wells No Violations I was on site for a follow-up inspection to the compliance notice issued on 9/22/2015 for this well. No changes have occurred since the last inspection. I spoke with Charlie Moseo owner of Plateau Energy Corp about when the well and tank battery clean-up efforts would begin. He indicated that he would be on site the following week to begin addressing the compliance issues.
1658658260 2018-02-14 Production Wells No Violations I inspected this well as a follow-up to Chiefs Order #2014-202 for failure to maintain liability insurance. At the time of this inspection, the well was being reported to be utilized by the landowner for domestic gas, but the well was not being produced commercially based on the closed flow-line connections. The was a pump jack unit and associated tank battery (comprised of one 100-barrel steel production tank, 50-barrel brine tank, with mechanical separator) Elements of the system appeared to be in disrepair and the flow-line connections were closed. There was remnants of visible oil in the tank battery containment from a prior release that had not been remediated. There remained no active liability insurance on file for Plateau Energy Corp.
