0274269027 2016-08-29 Production Wells No Violations Todays inspection was conducted to show Division of Oil and Gas Supervisor the well due to the ongoing review by the Divisions legal department and the Divisions field staff to determine if there is a responsible owner for the well. The current landowner, Coxon Investments, LLC, was not aware the well existed when the bought the property at a foreclosure sale. The well is in a pit with a manhole cover over the cement pit cover. The Ashtabula County Recorders office records show that a prior landowner, Pryately and Buchner, kept ownership of the well and the right to access and use the well prior to parceling off this property. They are currently attempting to sell adjoining property and ownership of this well through a land contract to Martin. The documents obtained from the Recorders office have been sent to the field supervisors and legal department for review to determine ownership.
1460556899 2016-03-15 Production Wells No Violations The Conneaut Fire Department, Tony Dernato, contacted the Division after receiving a call from Dominion Gas regarding high gas readings in the area where Dominion is working on replacing gas lines. Complaint # 5273. Dominion discovered a gas well in an underground 6 1/2 x 6 1/2cement vault in a vacant lot. The top of the vault is slightly above ground level. The fire department vented the vault by lifting the 24 steel manhole cover that accesses the vault. It appears a well is inside the vault, but due to the well being 4 below the top of the vault and debris and water in the bottom of the vault measurements of the casing are not possible. Slight bubbling is evident through the water and the gas scope showed 4% LEL/0 ppm H2S at the vault opening. A gas odor is evident. A rubber line is coming from the top of a fitting on the well and steel line with a valve is coming from the side of the casing. A step ladder and approved safety protocol will be needed for further inspection. Orange, plastic safety fence has been placed around the vault. The Fire Department said Dominion noted 3-4 domestic gas lines coming underground through the vault. The well is not registered. I contacted the landowner, Mr. Coxon, who had bought the property at a foreclosure sale. Mr. Coxon stated he did not know the well was on the property and may be interested in placing it in the Orphan program for plugging, if possible. Title work and further inspection will be needed.
1493640149 2017-04-21 Production Wells No Violations This inspection was conducted to follow up on the status of this unregistered well. Linda Pryately who is the owner of the well, but not the property the well is located on, was sent the paperwork to register the well by the Columbus Bonding Section. Rudy Pryately, Lindas brother, was also sent a copy of the requirements. Mr. Coxon owns the property where the well is located. Rudy stated that the family is reviewing the packet and will decide within the next three weeks whether to plug the well. I advised Rudy that the paperwork will need to be submitted and if the well is not going to be produced, the well will need plugged. The garage that the well is plumbed into and that is owned by Pryately/Buchner to the east of the well is now rented. The property to the north of the Coxon property that is also owned by Pryately/Buchner appears to still have a gas line plumbed to a shed on the property, the house burned down last year.
1496779811 2017-06-05 Production Wells No Violations Inspection of the well showed the garage on the adjoining property is still vacant. The second property that this well was used to supply gas to is torn down due to a fire. Both properties are owned by Pryatelys. The well is owned by Pryately but is adjacent to the above properties on property owned by Coxon. The well is currently unregistered. It was never permitted and was drilled years ago as a domestic well. I spoke with Rudy Pryately who is still deciding on whether to plug the well or register it with the Division. He would like until the end of next week to further research his options. This well was originally turned in as an Orphan Well complaint.
1529066462 2018-03-23 Production Wells No Violations I inspected the well location as a follow-up to previous inspections that had identified the presence of the well. Since intital inspections, there had been on-going dispute regarding ownership. At the time of this inspection, the well was located in a subsurface vault with lid structure and surrounded by a ornage snow-fence. The remainder of the parcel was undeveloped. There was no indication that the well was in current use.
1532540315 2018-06-22 UrbanProduction Wells No Violations I provided Mr. Pryately copies of land title documentation pertaining to the transfer of the property on which the well is located, and obtained from the Ashtabula County Auditors Office. Mr. Pryately had requested that he receive copies of these documents so that he could review. Mr. Pryately has disputed whether he, or others in his family, own the well.

REQUIRED REMEDIAL WORK: Plug well and The well owner/operator shall submit a written plugging schedule to Jerry Kohl at Ohio Department of Natural Resources Division of Oil & Gas Resources Management 3575 Forest Lake Dr. Suite 150 Uniontown, Ohio 44685-8116 within 30 days, per OAC 1501:9-11-05 [ B ], and plug the well within180 days. The well owner/operator will be notified if the plugging schedule is approve or denied.
