0015850695 2013-01-25 Production Wells Violations Noted The production tank was set less than 20 from the well head. The tank battery consisted of one 50 Barrel (Bbl) steel tank, one 2100 gallon (gal) plastic tank, and one water knock out. There was no dike around the tanks. The 2100 gal plastic tank had over flown with oil and brine and had contaminated an area around the tank battery roughly 30 x 12. The flow had moved into a field drain and moved through the field, and under the road through a culvert roughly 500 feet. There was no evidence of contamination reaching Sunday Creek. The company had already begun clean up but had not removed all the contaminants. All contaminated soils along the field drain and around the tanks shall be removed and disposed of properly. The affected area shall have soils replaced. The affected area shall be reseeded, mulched, and steps shall be taken to encourage vegetative growth. Any remaining contaminants shall be removed from the field drain.
0031434016 2014-09-24 Plug / Plug Back No Violations The 5 3/16 casing had parted at the collar last night, so the casing was set back down and screwed back on. The remainder of the bottom hole plug was placed through the casing and the casing was ripped at 277 feet and pulled using a packer. Clay is being washed down the 6 casing now.
0094279930 2013-01-25 Production Wells Violations Noted The production tank was set less than 20 from the well head. The dike shall be constructed with a capacity of at least 110% of the largest tank for the purpose of containing contaminants. I spoke with the owner in the field and we discussed the violation.

0911678336 2014-10-13 Administrative Inspection No Violations I received a call from MH Well Service concerning the clean-up of crude oil. The oil had pooled at the base of the sandstone rock face. The excavation was 6 deep. MH Well Service removed this material and continued to follow the stained soil until it was completely removed. The area has been seeded and mulched.
1243031964 2013-03-28 Production Wells No Violations Follow up to -15850695. There were still contaminated soils around the area of the tank and the field drain. I met with the producers and the drainage ditch was shoveled out and a small impoundment was made. Lime will be applied to the scalded area. The area below the dike on the hill will have the contaminated soil removed with in 2 weeks.

1585650626 2013-02-20 Production Wells No Violations Follow up to -15850695. Upon inspection I noticed not as much of the contaminated soils around the tank that leaked had been removed as I originally thought. There is still a small amount of oily vegetation along the banks of the field drain. There is still a small amount of mouse and sheen in the water in the field drain and it has now moved further along towards Sunday Creek. I was able to follow the sheen and mouse through the drain to Sunday Creek. There is no evidence of any recoverable amounts ever reaching Sunday Creek or any sheen along the banks of the creek but a small sheen was seen in the drain that empties directly into the creek. I spoke with Bruce Kelley and he will have someone out here as soon as possible to clean the sheen and mouse from the creek. Monday morning he and I will be meeting to discuss and begin remediation of the site.
