0527666671 2013-08-15 Production Wells No Violations Follow up to 1416250079. Identification on well, 4 1/2 casing equipped with a swedge and a closed 2 ball valve. There is a separator and multiple drips on site. A large oak tree has fallen across the site and landed on top of the 50 barrel steel tank, denting the lid and side of the tank. No production has been reported.
0592581293 2018-10-18 Plug / Plug Back No Violations The clay was still at the surface and the location was drying out. I called Nick with Kilbargers. I let him know the location was drying out and he said he would try to get people in there on Friday if it didnt rain.

I called Nick Vicerelli and he expressed concern with it being too wet to reclaim the area. I explained that it would be a good thing to leave the hole open and allow me to monitor the level of the clay and finish the reclaim when the location has dried out a little bit.

He agreed to have the old 40 barrel tank off the location and start work to lay a line to the horse barn for the land owners until such time as they can figure out a way to get to a sale line.

They were able to get telegraph at 2424 feet. They ripped the pipe and pulled 7 joints and placed about 4 ton of clay to cover up the top of the lime.
1299985660 2018-05-25 Production Wells No Violations Follow up to Chiefs Order to plug well. The well is still idle pictures were taken. I spoke to Nick Vickerelli a few days ago who stated they plan to appeal the order. There were no flowlines attached to the wellhead. The vertical separator was laying over on its side.

1416250079 2013-01-24 Production Wells Violations Noted The conductor and surface casing were open at the surface. The 4 1/2 production casing was on hangers and equipped with a swedge and a closed 2 ball valve. The 50 Barrel tanks has corroded away in many places but there is no evidence of contamination. Two drips and a separator are on site but inoperable. The identification on the well is incorrect. No production has been reported since 1991. All producing leases shall be legibly identified in a conspicuous place on or near the well-head or the storage tank as to owner, lease name, well number, permit number where available, county, and an emergency telephone number. The well shall be plugged or produced.
1416409619 2014-11-19 Production Wells No Violations Follow up to 1416250079. Work has been done to clean up brush at well site, but well remains idle.
1498609745 2018-10-01 Plug / Plug Back No Violations When I arrived today they were finishing sitting up the rig and getting the location ready to plug. Once they started shoveling clay the water came out of the 8 inch casing first, showing a hole in the 4.5 inch, somewhere. I asked them to shovel the 3.5 tons of clay in now before the rip to get as much down the 4.5 inch casing as possible. They finished up the day having placed 3 tons of clay in the hole.
1527707716 2018-05-30 Production Wells No Violations This well was originally issued a violation for O.R.C. 1509.12B. Today the wellhead is comprised of 4.5 casing, swage nipple to 2 with a valve in the closed position. The 4.5 casing is hanging on a casing hanger. There are no flowlines connected to the wellhead. The vertical separator is knocked over and laying on its side. This is the same state as I have found the well to be in previous inspections.
1531835806 2018-07-17 Production Wells No Violations The well remains in a state that is not capable of production. There are no flowlines connected to the swage nipple from 4.5 inch to 2 inch. The valve is in the closed position on the top of the swage nipple. I called and left a message with Nick Vickerelly the field supervisor with Kilbargers.
1556072327 2015-10-21 Production Wells No Violations Follow up to 1416250079. The conductor and the surface casing were open at the surface. The production casing was equipped with a reducer swedge and a closed 2 ball valve. There were no lines at the well. The 50 Bbl steel tank was still on site and full of holes, the separator was also on site but was laying on its side.

2099104217 2018-10-03 Plug / Plug Back No Violations Today I received a call saying the pipe that was ripped yesterday was stuck in the hole. Nick called me and said they pulled 45000 on it and wouldnt move, that was double the pipe weight. They were leery about pulling anymore than that because the location was very soft. I explained I was close and would be there in a minute. Once on location they let the pipe down and screwed in a short nipple and worked the pipe for about an hour and a half up and down. The pipe started coming and they pulled the 2424 feet of pipe. They then sat the pre-cast 7 inch plug on the 7 inch casing seat. Then they started the clay slurry.
