0270964109 2018-07-11 Production Wells No Violations The clean-up of the creek is going nicely. The creek is mostly clean to the first siphon dam. I instructed Jaime to clean up the leaves and debris that was oil soaked on the bank near the source to prevent further seeping into the creek. I took pictures of the siphon dams and spoke with Chris from OEPA this morning. He asked for pictures to be sent to him.

0377590587 2018-07-10 Production Wells Violations Noted Well is equipped with a pumping unit, tubing, and a traveling string of 1 inch diameter tubing that is being used as rods. There is an identification tag but it is illegible. I wrote the correct API and well number on one of the electric boxes. The flowline has parted and there is a small area of contamination. This will need remediated and the line fixed. Once I contacted the company and returned to the location I discovered the oil was in the creek. It traveled close to 1/2 a mile and I was able to get it stopped before entering Mush Run Creek. Once the clean-up began four siphon dams were installed to let the oil collect to allow for the removal of it. I discussed this violation with Jaime Metcalf owner of the well, in person while at the lease.

I sent Jamie Metcalf a text asking her to have her insurance company resend the required documentation to Bill Grubaugh. Pictures were taken.

0624794124 2018-09-12 Production Wells Violations Noted A small caged plastic tank used for cleanup is sitting 25-30 feet from the creek. This small tank will need emptied and moved. I texted Jaime Metcalf the owner operator of the company about getting the tank out of there.
0715692102 2012-12-19 Production Wells No Violations Follow up to -2031982932. Reviewed violations at well site with owner. We talked about bringing the well into production, putting an ID on the well, and removing the barrels of contaminants at the site.
0734509877 2018-08-01 Production Wells No Violations Follow-up inspection of the pollution and contamination violation found that the line that was apart close to the wellhead was repaired. The line where the worst of the contamination has also been repaired. There was one small area in the creek that still contained a trace amount of emulsified oil. The area on the bank where the release occurred still needs some onsite remediation. I called Jamie Metcalf and discussed the violation and also expressed concern about the large tree laying over the line. I explained I was not able to move the line and that the pressure from being pinched by the tree could cause the line to blow apart on top of the hill again. She said she would let her brother-in-law know about it and that Friday she would get out here and finish the remediation.

1970317988 2013-03-14 Production Wells No Violations Follow up to -2031982932. The 3 casing was equipped with a swedge and a 2 ball valve. The valve was closed, no lines, no pumping unit, No ID.
2031982932 2012-12-10 Production Wells Violations Noted Well not equipped with identification. Pumping unit was not attached to sucker rods and was falling of the support base. The 3 production casing was reduced and then closed with a 2 ball valve. No production has been reported on this well since 2006. Six 55 gallon drums were on site, 2 of which were full of fluids. One of the full drums was marked with a sticker indicating the contents to be Prohib II, which is an acid used to treat wells. All producing leases shall be legibly identified in a conspicuous place on or near the well-head or the storage tank as to owner, lease name, well number, permit number where available, county, and an emergency telephone number. Well shall be plugged or produced and records shall be submitted to the Division of Oil & Gas. All equipment possible containing hazardous materials shall be removed and disposed of properly in order to prevent possible future contamination.
