2018-01-25 | Production Wells | Violations Noted | Went to the location of a release self-reported by the company. They reported that they lost the bottom of the Drydock tank battery and lost approximately 40 barrels of oil and 3-4 barrels outside the dike. The release happened the afternoon before on the 24th of January. I arrived early on the 25th before the crews were on site. I was accompanied by Inspector Brandon Mitchell. I found the site to be like it was reported the previous evening. The location was left in such a way that the oil was contained and left for cleanup during daylight hours. I spoke to Benny Altier, Todd Altier and met with Bruce Fouts on location. We went over the general plan to achieve the best possible results with the clean-up.
2018-08-16 | Production Wells | No Violations | The tank was properly identified. Violation is resolved.
2001-05-01 | Annular Disposal | Violations Noted | FOLLOW UP TO NOV # 5609i177 An inspection of the well revealed there is No ID at the well head, and ID needs to be placed at well head.
2018-05-11 | Production Wells | No Violations | The pollution and contamination has been cleaned up. I am just waiting for the dike to be built.
| 0920595833 | 2004-04-06 | Production Wells | No Violations | No violations at the time of inspection.
Updated well latitude and longitude. Recorded coordinates of the tank battery location. |
2018-01-31 | Production Wells | No Violations | I went to Altiers shop today and spoke with Todd about the next steps. I explained that if his choice was to bioremediate the soil it would need to be lined and a berm around the outside. I also explained that I would need to know the location and that I would inspect it to make sure it was lined and bermed. I then explained that they would need to try to achieve the 50 foot setback requirements for the new tank they set. If they could only get 45 feet I would approve it if they would set a few more posts parallel with the road. They explained that their goals for today was to get the tank set and the wells back into production today since they have houses connected to the wells for heat.
2018-04-12 | Production Wells | No Violations | The dike is still not completely rebuilt. I called Benny Altier who stated, the dike should be completed at the beginning of next week.
2008-12-22 | Annular Disposal | No Violations | The well was not equipped for annular disposal.
2018-07-25 | Production Wells | No Violations | The dike drain has been installed. The dirt at the location still needs to be brought to a height capable of holding the contents of the tank. There is also no identification posted at the wellhead.
2018-01-31 | Production Wells | No Violations | I returned later that day and found that the affected soil was piled into the dike ready for disposal and the free oil outside of the dike was reduced to approximately 30 gallons that was contained with a small earthen dike.
2015-10-19 | Production Wells | Violations Noted | No ID was found at the well head or on the old production or drop tank. The lead line going to that tank has the valve closed so it is not currently being used. It appears the well has been serviced within the last few years. An oily mess was found at the well head covering a 5x20 area. There is also a kill zone associated with the referenced oily mess indicative of brine. The mess needs cleaned up around the well and some stone would suffice to cover the area next to the well head. However the area down hill toward the orange pipe and hay bales would need seeded and mulched. ID needs posted at the well head in a manner that is consistent with Ohio Oil and Gas laws and rules. The area at the back of the tank where the lead line, that is not currently being used, was leaking needs cleaned up and contaminated soil removed. Fresh oil was noted on the casing head, the source of the leak, adding to the oily mess around the well head needs corrected.
2015-12-04 | Production Wells | No Violations | All NOVs from inspection 1648544420 have been corrected. The tank is still present and will eventually need to be moved. However no other violations noted at the time of this inspection.
