1019508007 2013-04-02 Production Wells No Violations Follow up to inspection 1218780322. The first dike has had the water and the contaminated soils removed, the area has been seeded and mulched. The three small areas outside the dike have not been remediated. The second dike has been emptied of fluids and one of the tanks has been moved. The contamination below the tank with the leaky valve has been mostly removed. The tank does not have a plug in it yet. The third dike had been emptied of contaminated fluids and rebuilt. At the time of inspection it contained rain water that was field tested with a quan-tab and found to have less than 28 ppm Chlorides. The contaminated soils and vegetation at the third dike had not yet been remediated.
1218780322 2013-03-26 Production Wells Violations Noted The primary dike at the main tank battery contained production fluids and oil. The dike contained two 100 Barrel (bbl) steel tanks and one separator. The dike was roughly 30 x 25 and had only two walls, each on the down hill side. The fluids had puddled in the lower part of the dike. Fluids had seeped outside the dike in three small areas, each roughly 2 x 2. A second dike at the main tank battery was roughly 20 x 20 and full of water which was field tested with a quan-tab and found to have 53 ppm Chlorides. This second dike was associated with two steel tanks, one was a 100 bbl tank and the other a 50 bbl tank. Neither of these two tanks were connected by any lines but roughly 2 gallons of oil had been released from one of the tanks from the gate valve contaminating an area 15 feet long. Neither of these tanks had the required locks or bull plugs. A third dike was found on the back side of the lease and also found to have oil and production fluids in the dike. The third dike was 12 x 18 and had one steel 100 bbl tank. The steel 100 bbl tank was not set in the this dike, but above it on a hill. Oily materials had coated the ground and vegetation in the area and a 2 plastic load line with a quick connect was found nearby that was plumbed into a 2 steel line that ran in the direction of the main tank battery. No violations at the well head. All oily materials and contaminated soils shall be removed and disposed of properly. Any affected areas shall be reseeded, mulched, and steps shall be taken to encourage vegetative cover. All fluids shall be removed from dikes. Defective dikes shall be rebuilt in order to contain fluids properly. All tanks shall be equipped with locks or bull plugs. All equipment shall be rated to withstand normal operating pressures.
