0992392917 | 2018-06-15 | Salt Water Injection Well | Violations Noted | At 1325 hours I received a call from ODNR emergency response team member regarding a brine release associated with this well. I arrived on site at approximately 1330 hours. I met with Project engineer Mr. Robert Rudolph who showed me the source of the leak. The leak came from the packing nut on a valve that was plumbed in line near the wellhead. This leak was detected approximately 10 minutes after it starting leaking. Mr. Rudolph met me and showed me the extent of the brine release. The brine was found in the gravel that was surrounding the wellhead. It also had escaped the gravel area and ran down hill into a low spot. K & H Partners had a vacuum truck on site to remove any free brine. I was accompanied by Mr. Nate Lane with ODNR emergency response. Mr. Lane used a soil conductivity meter to determine the extent of the brine spill. The extent of the spill area was approximately 18 feet wide and 40 feet long. The results of the soil conductivity meter ranged from 50 ppm to 2000 ppm. The brine that was in the gravel had a sump installed to allow the brine to be removed at the sump. The brine that was on the hillside was removed and placed in a lined container. The brine that was at the low point is being collected in a plastic lined sump. Photographs were taken during my inspection. |
1072993857 | 2018-06-20 | Salt Water Injection Well | No Violations | Compliance notice # 9923929 follow up inspection. Meet with Nate Lane ODNR-DOGRM ER and Rob Rudolph on site. The gravel area at the wellhead has been excavated and has a built up clay containment and is to be flushed with fresh water and then collected by a vacuum truck for disposal at the K&H facility. There are 8 roll off boxes on location. |

1181483055 | 2016-03-08 | Salt Water Injection Well | No Violations | Phyliss Rhinehart called to complain about excessive noise and vibrations that were shaking her floor. I explained that I was with someone from a company showing them locations of wells and would get there as soop as possible. To talk with them personally. Her husband then took the phone and explained that he went and parked in the middle of the entrance for the well but no one would stop and talk to him. I explaiend that was dangerous and not a great idea since it could get a officer invovled. I explained that I would be there as soon as possibble again. |

1389325401 | 2014-01-09 | Drill / Deepen / Reopen | No Violations | Witnessed 96 feet of new 13 3/8 conductor set in 122 feet of 17 1/2 borehole with 114 sacks Class A cement. Circulation established and good returns seen at surface. Cement fell back roughly 10 feet and was grouted to surface using 4 sacks of Class A cement. The cement top fell again roughly 10 feet. Not enough cement on truck to top off again, will check cement top tomorrow after it has set. |
1389731861 | 2014-01-13 | Drill / Deepen / Reopen | No Violations | Witnessed 379 feet of new 9 5/8 surface casing set in 410 feet of 12.25 borehole with 141 sacks Class A cement. Circulation established and good returns seen at surface. During the casing pressure test roughly 2-3 Bbl of cement was burped out between the annular space and the cement top dropped an estimated 100 feet. 28 sacks of Class A cement were used to grout back to surface. The cement top continued to fall 2-3 feet while I was still there. Casing pressure test did not indicate a failure in the integrity of the surface casing string. |
1389744570 | 2014-01-14 | Drill / Deepen / Reopen | No Violations | Witnessed remedial grout job on surface casing. A spaghetti sting was ran down the annular space and 9 sacks of Class A cement were circulated to surface. Good returns seen at surface and did not fall back. |
1389796816 | 2014-01-15 | Drill / Deepen / Reopen | No Violations | Administered surface casing pressure test before drilling out the shoe. Minimum test pressure was 300 psi, actual testing pressure was 390 psi. A 15 psi loss in pressure was recorded over the 30 minute testing period which is less than the allowed 10% loss to prove casing integrity. |
1390058718 | 2014-01-16 | Drill / Deepen / Reopen | Violations Noted | Upon arrival at the site at 9:15 am I noted a vacuum truck on site that was drawing water from the pit. I spoke with the driller and he informed me they had hit water in the Injun Sandstone at 1432 feet and the pit had started to take on water. He also informed me the pit had begun overflowing shortly before I arrived, around 9am. An estimated 40 Barrels of formation water had left the pit and crossed the location and entered a small intermittent stream nearby, roughly 60 feet away. I noted drilling soap foam in sections of the creek and that the pit water was currently entering the stream. A section of the stream was dammed with soil near the source of the contamination and again roughly 250 feet downstream from the source with straw filled erosion control socks. Water samples were taken at the source, 250 feet downstream from the source, and in a separate upstream fork of the stream roughly 50 feet from where the two stream converge as a control sample. Drilling operations shall cease until further notice by the DOGRM. All contaminated materials, including drilling fluids and soil, shall be removed from environment and disposed of properly. Drilling location shall be isolated from environment by a secondary containment system in order to prevent further contamination. |

1711096331 | 2016-03-08 | Administrative Inspection | No Violations | Jon Scott returned my call and I briefly explained what was going on. He gave me a contact number for Rob Rudolf the facility manager. I called Rob and explained there was a complaint for excessive noise then asked if there was anything going on outside of ussual activites at the site. He then said they were sucking out a tank bottom and that the pump was very noisy. He appologized and said it was something that does not happen very often maybe 2 or 3 times a year. I thanked him and then called Phyliss back to explain they were just sucking out a tank and that is why the noise was coming and going. She thanked me for the information and following up with them. |
