0029466834 2010-12-29 Plug / Plug Back No Violations Inspection this day due to continuing of monitoring of plugged well that had been resealed and vented in residential neighborhood. It was a clear day and no gas was observed coming from vent pipe. Will continue to monitor for leaks.
0295168732 2010-09-08 Orphan No Violations Today, dug out hole around the wellhead by hand, trying to find gas leak. Well was bubbling thru the water on outside of casing. Cut casing off and found well bubbling inside casing as well as outside, about 3 to plug inside the casing. Put plumbers temp plug inside 4.5 casing, to seal off gas leak inside casing in hopes of finding the leak outside of the casing. Gas continued to bubble around outside of casing after adding water to the hole. Have concluded that gas leak too great to just fill in hole or to just throw additional cement on top of - must dig it out with a backhoe and will bring that and dump truck tomorrow with goal to find where leaking outside of casing, seal it off and reseal inside of casing where leaking.

1144702581 2010-09-09 Orphan No Violations Today work began with backhoe digging the hole bigger around the well. This proved that wellhead/casing was indeed loose, just laying in there and pulled it out with backhoe. Found wellhead came out freely, exposing an open hole with 12 wooden casing surrounding the well. Hole is bubbling more with gas than before and bar finds there is resistance/plugging at 6. As work the bar in the hole, find it is bubbling more than before. Large 18 steel casing 9 long was set over the well, centering it and then driving it down over the well to a depth of @4. 18 casing now stands 5 high and this space filled with 6 sacks of Quikcrete, then filled with dirt for a total of 4 of fill. Steel cap was welded onto 18 casing with nipple in center to connect 1 vent pipe onto, leaving 10 of vent pipe above the ground level. Permit number was etched on cap, then hole filled in and packed. Location was seeded and mulched next day. Once the cement sets up, leak should be gone but if it persists, will be vented out of vent pipe. Will monitor well for next few months to year to see if leak is still present. If there is no gas present, it is suggested vent pipe be removed and capped at ground level. If the well has to be reopened and cleaned out, the 18 casing should work well as conductor.
1156460869 2010-08-17 Orphan No Violations Gas leak discovered by Dominion in residential area of Athens when walking their lines for leaks. Hole dug exposed an old gas well 5 below ground level, 21 from edge of house and 15 from edge of street. Dominion notified Athens City Engineer and owner of home - ProRentals. Street was reportedly developed in 1906 and well appears very old, probably 100 years or more. Completed with 4.5 or 5 casing, well is sitting on a clamp and appears to have clay around outside - suspect there is surface casing covered yet with dirt. Well has 5 x 1.5 swedge with 5 long vent pipe coming above it - capped with loose plug - that lays at ground level. There is a 1/2 hole in the swedge and gas is escaping at very low rate - when you cover the hole, pressure will built and can hear gurgling with waves of gas coming out of top of uncapped vent pipe. Old gas lamp about a foot away from the surface exposure of the well indicates that well probably ran the gas lamp and more in the neighborhood. House may have been built shortly after street developed. Old maps show this location was the end of the street at that time. House sits about 10 above the street with a gentle slope down to the street - there are steps up to the house near the well and a retaining wall below and between the well and street. Location is 103 Franklin St., owner of house is ProRentals. Neighborhood is many small older homes used as rentals for college students. Street is narrow with one-way travel, many overhead wires supplying cable, telephone, electric, and more to the houses that will contribute to the expense of plugging this well. Also sewer and gas lines in close proximity to well. Because well is 5 below the ground surface and on same level as street, much dirt will have to be removed to servie well. No records in area - closest well is old Berea at 1133. Advised owners to put fence around, NO SMOKING signs, plywood over hole until well can be temporarily fixed to become safe. Talked to Mi

- now believe well probably plugged and that is why well top @ 5 below ground level. With lighter, lit well and did burn about 2 high - continued to burn, not changing in strength. Will ask Columbus for futher instructions, suggesting to cut off wellhead and vent pipe and fill 2 open in hole with cement, then see if well still leaks. Only other choice would be to drill out well and re-plug. 8/24-

1283364785 2010-09-01 Orphan No Violations Inspection this day found that black water had been bailed down in hole to threads on collar of casing, still small amount of water in hole but does not appear to be making water. Does not appear to be sewer water and City of Athens says they tested their water and sewer lines and found no leaks. Gas is now bubbling up out of water in 3 places around the outside of casing, vent pipe still on well. Will contact Mike McCormac to see what he wants to do next. Conclusions are that it was old gas or water well with gas in it, has been plugged and is leaking. Need to fix problem and get hole filled in in next few days.

1940884392 2013-08-28 Production Wells No Violations Associated with complaint # 4435. Landowner across street was concerned about safety of plugged/vented well. She reported last year durring winter break ( winter of 2012) the tree in the yard less than 20 feet from the vented well caught fire. Tennants in house explained tree had caught fire from below ground level, had smoldered for roughly two days, and then burnt from the inside out. Fire department was called to extinguish flames. No elevated levels of gas on meter. There is a small amount of gas being released from the vent pipe, roughly 12 in the air. The burnt tree stump lies in between the gas meter on the house and the street. The tennants were encouraged to contact Columbia gas to inquire about an inspection of the gas line to their house, and if it may be related to the trees combustion.
