0056926169 2011-06-16 Salt Water Injection Well No Violations An inspection conducted with Mr. Will Ziegler revealed no violations at this time. During the inspection, it was noted that the Pre-liminary Restoration meets ORC 1509.072. The facility did not contain a concrete vault or unloading pad, as water is not hauled to this location. Identification was observed at the well. The injection pump was not running at time of inspection and positive pressure was not observed with gauge on the annulus.

Cleanup efforts still conducted by Allied Envronmental under the supervision of Ohio EPA and AuglaizeCounty EMA.

1358867689 2013-01-12 Production Wells Violations Noted At the time of my inspection I was called outbecause of a release of oil on site. The high level shut off switch in the oil tank had failed around 10:30 pm on 1-11-13 and caused the tank to overflow into the containment dike of the tank battery. A submersibal pump with an automatic switch was in the dike to remove rainwater and came on when the dike filled with oil. The setup of an automated submersible pump was not in the operating plan nor was it approved by the Division of Oil & Gas as indicated in the Permit Application. This put oil on the ground outside the lined dike. The oil eventually made it into a field tile then to a ditch 400 to the east. The oil then flowed 1000 south to where the ditch empties into the East Branch of Clear Creek. The high level of Clear Creek at the time prevented any flow from the ditch into Clear Creek so it is believed that no oil made it past this point. Cleanup efforts were conducted by Allied Environmental under the supervision of Dave Schilt, Ohio EPA, and Troy Anderson, Auglaize County EMA. Cleanup efforts were completed, with the exception of the monitoring of the ditch, by 1-16-2013. Officials from Innex have indicated they intend to plug and abandon all three wells on this lease.
1831819915 2013-05-21 Final Restoration No Violations At the time of the inspection all plugging operation were completed and all equipment had been removed from the location. The location is in a farm field and was graded by the plugging contractor. The location was tilled and planted by the landowner. Final Restoration Passed.

RBDMS currently shows well status as DR.
1989843351 2011-11-21 Production Wells No Violations Well is shut in and being serviced. ID posted but has incorrect API #. Sign is posted as 20103 and needs to be 20102. Signs on both wells need switched with each other. I talked with operator who will do so when wells get serviced.
2125792414 2013-04-03 Plug / Plug Back No Violations At the time of the inspection a bottom plug was set on top of the bridge plug (set on 4-1-13 at 1210) from 1200 to 970 with 40 sacks of 50/50 pozmix from PetroSet Cementing Services. The cement was pumped through 1.5 tubing and no circulation was observed. The cement will be tagged with the bailer after 24 hours to determine the top of the plug.
