There is a riser pipe with a pressure relief device next the well head that appears to be attached to the casing well head that is not above ground level. The well is set up to be produced by the means of a pumping unit with a electrical drive motor. On location near the well head is the production facility equipped with (2) production tanks, one plastic capable of holding 80 barrels and one steel capable of holding 100 barrels. During previous inspections the producer has been cited for pollution and contamination due to production fluid being discharged and on this day a brine scald was observed - measured and photographed below a drain outlet associated with the production facility. The scald area measured 16 x 16 and was Quantab tested for chlorides with a high level Quantab strip test and showed 1500 parts per million chlorides. Photographs taken on this day and I spoke with my supervisor Brent Bear regarding the scald area below the production tanks. |

1427123541 | 2015-03-23 | Production Wells | No Violations | Status check to assist inspector Andrew Corder in response to a complaint received by the landowner George Lafaber. Upon inspection on this day the following was found at the wellhead 6.625 casing 2 tubing and pumping rods with both the production and tubing valves open and production flowlines present. At the production fluid storage area a single 100 barrel tank was found present with a fluid separator located on top of the tank. In response to the complaint of pollution & contamination a discharge flowline was observed coming off the fluid separator and feeding into a 3 black in color corrigated line. At a measured point of 62 from the tank in a defined ditchline a 3 black in color corrigated line was found at a GPS point of 39.87166 x 81.08766 with stressed vegetation and visible scalding present to a measured point of 167 down the ditchline below the discharge point of the 3 line. Quantab Chloride low @ high level field strip tests were taken at the entry point and readings were found to exceed both Quantab strip tests, at the last measured point in the ditchline readings were also taken and found to exceeded the low level Quantab strip tests and read 5,737 parts per million on the high level Quantab strip tests. Photographs were taken on this day. |

I let him know that dispersal was acceptable under Mississippian Exempt law however we had to be sure that this well meets all the criteria. There is an area of stressed vegetation below the production facility, the area was Quantab tested for chlorides with a low level Quantab strip with the strip test indicating 247 parts per million chlorides. The producer Jim Yontz was contacted and asked to establish proper vegetation in the area as weather permits. |
