A single 100 barrel production tank is present on location near the wellhead with a fluid knock out separator located on top of the production tank.

1510179281 2017-11-08 Production Wells No Violations Status check inspection as a response to a complaint filed by the landowner Diane Kinney who indicated that a well tank on the property was overflowing and leaking into a nearby stream. I arrived at 13:00 pm to assist the lead County Inspector Andrew Corder and ODNR Emergency responder Jason Blocker. Upon inspection of the well the well was observed to be equipped with 4.5 diameter production casing with a 2.875 diameter inner isolation string and 1.900 diameter production tubing. There is pumping rods inserted inside the production tubing and the only production flowline present is attached to the production tubing head with no valve on the tubing flow line. The well is set up to be produced by the means of a electrically driven pumping unit set on a automatic timer. On location near the well head is the production facility consisting on 2 production tanks one being of steel construction and one being plastic, both tanks are of the same approximate capacity of 100 barrels volume and the steel tank was observed to be overflowing with what appeared to be the same consistency of crude oil. Both tanks were situated inside a small earthen dike that was free from standing fluid, also noted was the oil running from the tank could be seen running to the lowest elevation point in the dike and not staying above grade level. Directly below the production facility is a wet weather stream bed that has flowing surface water currently present with a visible oil sheen on the surface. I place oil boom filter socks in 2 locations and oil absorbent 2 x 2 pad downstream for approximately 100 yards to stop the down stream migration of product from the well. Also noted was at a location in the stream be of a coordinates of 39.86869 X 81.08365 oil could be seen entering the stream from what appeared to be a underground source later identified as a 4 corrugated plastic pipe that was traced back to the inside of the tank battery dike area low elevation area. 1510758686 2017-11-08 Production Wells Violations Noted I received a call from Scott King of the Emergency Response Group regarding a call that was received on this well. The call came in from Diane Kinney, who is a landowner nearby the well. The caller alleged that there was a noticeable sheen in the nearby stream. I arrived on scene and was accompanied by Inspector Bill Stolarik and Emergency Responder Jason Blocker. Present for the Division of Wildlife was Officer Wesley Feldner and Officer Supervisor Bryan Postlethwait. Present for the Ohio EPA was Aaron Wolfe. 1538415777 2018-10-01 Production Wells No Violations Follow up inspection to a violation issued on 11/08/2017 By Andrew Corder. Upon inspection the well head was observed to be equipped with a 4.5 diameter casing head along with a 2.875 intermediate casing string and a 1.5 diameter production tubing string with a 1 diameter tubing line attached to the tubing string only. The 2.875 diameter casing head has a pressure relief device installed. The well is set up to be produced by a pumping unit equipped with an electric motor set on a mechanical timer. On location near the well head is the production fluid facility consisting of (2) production tanks one plastic and one steel. The plastic tank appears to be 80 barrels fluid volume capacity and the steel tank is 100 barrel capacity. The plastic tank is full and the steel tank is empty. Since my last visit the producer has remediated the affected area caused by the last spill incident. The violation is considered resolved and I will send notification to Supervisor Mike Moreland and request the violation be closed.
