0313239511 2016-03-14 Preliminary Restoration No Violations On this day I visited this pad to perform a preliminary restoration inspection. The pad appeared to be in satisfactory condition. Vegetation has been established and the erosion and sediment controls appear to be functioning properly. Preliminary restoration for this site has passed.
1172219140 2017-09-26 Production Wells No Violations This well is equipped with 5.5-inch diameter production casing. The well appears to be free flowing without any artificial lift technology present at this time. The well is piped to a gas processing unit and then to the production area. The production area contains three 400-barrel production fluid storage tanks and one 500 barrel storage tank. The production records within the RBDMS system are current to 2017.
1441032348 2019-03-18 Production Wells No Violations Status check inspection found 3 wells present on the pad. All 3 have been drilled and completed. The said well does not have artificial lift technology installed on the production tree. The 5.5 production casing pressure is 500 pounds. Annular pressure monitoring is installed on the well head A&B sections. There is 3 gas processing units present on the pad. The production facility consists of (3) 400 barrel and one 500 barrel steel production tank situated inside a polymer lined steel containment dike area. Proper well identification is installed on the well head with hazard warning labels on the tank battery. The well pad and lease road surface is stone construction and in good repair, the well pad perimeter is protected by an earthen dike.

1927696233 2018-09-21 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with 5.5-inch diameter production casing, and 2.875-inch diameter tubing that appeared to be open flowing. Annular pressure monitoring was in place for the 9.625-inch diameter intermediate casing gauge read 220 pounds per square inch, and the 13.375-inch diameter casing gauge read 0 pounds per square inch. Production equipment included a Sand Separator, Gas Processing unit and 3 400-barrel production fluid tanks, and a 500-barrel condensate tank shared with the other producing wells on the pad. There were multiple Emergency Shutoff Devices on location, Identification tags were present, and production was up to date.
2068240203 2014-02-20 Drill / Deepen / Reopen No Violations On the day of my inspection Nomac 311 was stuck in hole. They were going to try and jar on the drill pipe. No violations noted.
