0029845051 2017-09-26 Production Wells No Violations This well is equipped with 5.5-inch diameter production casing. The well appears to be free flowing without any artificial lift technology present at this time. The well is piped to a gas processing unit and then to the production area. The production area contains four 400-barrel production fluid storage tank. The production records within the RBDMS system are current to 2017.
0210873281 2014-03-19 Drill / Deepen / Reopen No Violations On the day of my inspection Nomac 311 was rigging up. No violations noted.

0874555657 2018-07-03 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with 5.5 inch diameter production casing, and 2.875 diameter tubing with a plunger lift system. Annular pressure monitoring was in place the 9.625 inch diameter intermediate casing gauge read 210 pounds per square inch, the 13.375 inch diameter surface casing gauge read 50 pounds per square inch. Production equipment included a Sand Trap, Gas Processing Unit, 4- 400 barrel stock tanks and a 50 barrel produced water tank shared with 34013207580000, 34013207590000. Identification tags were present, and production was up to date. No violations were found at the time of my inspection.
1202372207 2014-07-03 Completion Testing No Violations On the day of my inspection completion operations were taking place. All wells are being fraced at the same time. (zipper frac). No violations noted.
1209862531 2016-09-07 Production Wells No Violations During this inspection I was accompanied by Austin Gutridge. This well is equipped properly to be considered capable of production. The piping from the well leads to a gas production unit then to the tank battery. The tank battery consists of four 400 barrel production fluid tanks. This site appeared to be in satisfactory condition.
1214157635 2015-09-10 Preliminary Restoration No Violations On the day of my inspection the well was properly equipped to be considered capable of production. The tank battery consisted of four 400 bbl oil storage tanks. There was a separation area on site. All units were in containment and the containments areas were reasonably free of liquids. The erosion and sedimentation controls were of satisfactory condition and proper vegetation had been established. The preliminary restoration for this well has been completed.

1546208993 2014-08-11 Completion Testing No Violations On the day of my inspection I tested the sump area for elevated chlorides. I also checked surrounding puddles. I found that there was still a presence of elevated chlorides and I will conduct further investigation later this week with a company representative. We will determine the best remedial action at that time and proceed.
